HomeMy WebLinkAbout992567.tiff RESOLUTION RE: APPROVE GRANT AGREEMENT/CONTRACT AND AUTHORIZE CHAIR TO SIGN - COLORADO ACTION FOR HEALTHY PEOPLE WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Grant Agreement/Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Family Education Network of Weld County, and the Colorado Action for Healthy People, commencing October 15, 1999, and ending October 14, 2000, with further terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Grant Agreement/Contract between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Family Education Network of Weld County, and the Colorado Action for Healthy People be, and hereby is, approved. If you have any questions please contact Tere Keller-Amaya at extension 3342. 992567 COMMUNITY ACTION rb :HEALTH PROMOTION INITIATIVE FUNDED BY THE COLORADO TRUST ADMINISTERED BY COLORADO ACTION FOR HEALTHY PEOPLE GRANT AGREEMENT/CONTRACT Grant Number: HHAB-99TR-103 Family Education Network of Weld County(hereinafter"grantee"), a nonprofit organization, hereby accepts the awarded grant amount of$10,000 from Colorado Action for Healthy People (hereinafter"CAHP") to conduct a child healthy habits project. The term of this award is from October 15, 1999 through October 14, 2000. Final financial and progress reports will be due on November 15, 2000. The grantee agrees to administer the grant in compliance with their proposal submitted to CARP (which is hereby incorporated as part of this contract as"Attachment A") and the following provisions: 1. Use of Grant Funds: This grant is restricted to the purpose specified in Attachment A and is to be spent in accordance with Grantee's proposed budget. Any part of the grant funds not so used within the term of the grant, must be returned to CAHP within 30 days after the completion of the project or upon expiration of the grant. 2. Local Support: Grantee agrees to provide local cash and/or in-kind support in the amount indicated in the budget section of Attachment A. 3. cost Principles. a. Grant funds will be used only for project purposes set forth in the original application submitted to CAHP, unless changes are subsequently approved by CARP, as explained in#5 below. b. Grant funds must be obligated during the grant period. No grant funds may be applied to any project expenses incurred prior to the date of this agreement. 4. Grant Accounting. and Records: a. Grantee agrees to maintain adequate accounting records and to obtain documentation for all grant expenditures. b. Grantee agrees to document all local support. Grantee agrees to provide other reports and meet all other financial requirements as established by CAHP. c. Grantee agrees to maintain records and accounts consistent with generally accepted accounting principles, and to provide for such fiscal control as is necessary to assure proper disbursing of, and accounting for, project grant funds. The Grantee certifies that accounts and supporting documentation relating to project expenditures will be adequate to permit an accurate and expeditious audit. An audit may be made at any time by CAHP, its designated representatives, or any appropriate agency of the United States government. 5. Project Changes: Any significant change in project activities or expenses must receive prior written approval from CAHP. These changes include but are not limited to changes in the project's scope, purpose or activities, changes in expenses of more than 20 percent of an approved budget category, and changes in the duration of the grant period. 6. Payment of Grant Funds: Awards of CAHP funds will be made in the following manner, unless otherwise negotiated with CARP: a. One half(50%) of the total grant shall be paid to grantee by October 31, 1999, contingent upon receipt of the signed grant agreement, and approved reconciliation of all CARP funds awarded in a prior grant year. b. The second payment shall be made in an amount up to an additional 2.5 percent of the total grant upon receipt of a signed reimbursement statement and a six-month progress report (formats to be provided by CAHP). The reimbursement statement shall include dates, amounts, brief description of the expenditures and the total requested. Original receipts or other supporting documentation shall be kept by the Contractor. c. The remainder of the total grant (25 percent) shall be paid upon receipt and approval of the final reimbursement request and the final progress report. d. Indemnification• Grantee agrees to indemnify and hold harmless Colorado Action for Healthy People from any and all actions, claims, demands, costs or judgments arising out of or in any way connected with Grantee's use or expenditure of the monies paid to Grantee under this Agreement. This section shall not apply to state, county, city, town, township, or school district grantees. 8. Termination: In the event that the Grantee fails to meet the terms of this contract or if in the judgment of CAHP the Grantee becomes unable to carry out the purposes of the grant, CARP may terminate this Agreement at any time prior to the expiration of the grant. If this Agreement is so terminated, Grantee shall, within 30 days of the effective date of termination, repay to CARP all unexpended grant funds. Grantee shall provide to CAHP a full and complete accounting of all receipts and disbursements of funds and expenditures incurred under this Agreement through the date of repayment to CAHP. Acknowledgment: The Grantee will acknowledge CAHP's funding on any material developed through this grant. Number: 84-6000-813 Page 3 of 3 pages ATTACHMENT A Project Summary Moving Our Children 1999-2000 Family Education Network of Weld County The Moving Our Children program will be introduced to our four remaining sites during the 99/00 school year. These new sites include Frederick - one classroom, Platteville - one classroom, Hudson - two classrooms and East Memorial in Greeley- 2 classrooms. They will receive 10 training classes, 3 workshops and the same curriculum materials that were presented to our 8 previous sites. They will also receive three new audio tapes produced by Funfit and 2 Diamond Balls to facilitate classroom activities. The inclusion of these sites will complete the training for all the teachers and aides involved in our Head Start community. The eight sites that participated in the previous two years of training will receive 6 booster training's to keep these teachers motivated and further strengthen their teaching skills. They will also receive three new audio tapes produced by FunFit, and two Diamond Balls to use with their children in the classrooms. These teachers will also attend three in- service workshops to be presented during the year. Each of the four new sites as well as the eight previous sites will receive two parent workshops during the year. In addition to the workshops, all the parents in our community will receive four newsletters throughout the year. As in previous years, these newsletters will explain the benefits of maintaining active lifestyles, present activities and offer prizes for parent participation. We will also produce a new instructor's video for each of our teachers. This new video will show teachers how to use simple manipulatives like paper plates, balls, scarves, sticks and bean bags to further enhance their children's strength, balance, endurance and coordination. At the end of this school year we will have successfully trained all the teachers and aides in our entire Head Stan community. This training and the curriculum materials will allow them to continue promoting physical activities and active lifestyles to our children in the future Goal We willpromote and affect the healthy habits of our children by reducing the known risk factor of physical inactivity that could contribute to'chronic diseases in adulthood Objective #1 By the end of June 2000, 80% - 100% of the children who participate in this program will experience enhanced motor skills, strength and endurance while demonstrating more knowledge and enthusiasm about the importance of physical activity. Steps to accomplish this objective: 1. Beginning in October four new sites, as well as the eight previous sites will provide their children with 20 minutes of developmentally appropriate fitness activities each day. The teachers will receive proper training as outlined in Objective # 2. 2. Posters promoting active lifestyles will be placed in the classrooms and children's handouts will be distributed periodically. 3. As in previous years, children will be measured for improvements in strength, balance, endurance and attitude. 4. Children who participate in these activities at home with their parents, as outlined in a parent newsletter, will receive prizes that further promote physical activity. (for example; Playtangle Kits, Adventurobics Audio Tapes, Balls, Scarves etc.) Measurement of Objective #1 Accomplishment Children will display improved proficiency and enthusiasm when performing physical activities, as witnessed and documented by classroom teachers. They will be more willing to display their skills by observed utilization and practice during freeplay. Motor skills and self esteem will be improved as indicated by our assessments. Along with improved motor skills, children will demonstrate observable changes in knowledge, attitude, and behavior towards physical activity by demonstrating more body awareness and encouraging others to exercise. Question and answer sessions, provided in the curriculum materials will demonstrate children's increased knowledge. Like previous years, our assessments and evaluations will show how children have improved in physical strength, balance, endurance, coordination and self esteem. Objective #2 Throughout the 99/00 school year and beyond, 80-100% of the teachers and aides who participate in this program will be competent and eager to promote physical activity and incorporate a daily curriculum of developmentally appropriate activities in their classroom. Steps to accomplish objective #2 I. Beginning in October the teachers and aides at four new sites will receive training in their classrooms with children present and participating. These sites will receive 10 training classes by FunFit throughout the year. They will also receive curriculum materials provided by FunFit and Front Row Experience to help them facilitate the classroom activities and their objectives. 2. Last years participating sites will receive 6 "booster" training classes in their classrooms throughout the year. They will also receive three new audio tapes by FunFit: and Diamond Bails from Front Row Experience. 3. Three in-service workshops will be provided throughout the year to keep teachers informed of updates and allow them to express concerns and observations in regards to the training classes. 4. The instructional videos and worksheets that were developed two years ago will be distributed to all the teachers. 5. Activity checklists will be given to the teachers to keep track of their involvement and assure that they are presenting the activities to their children. 6. A new instructor's video will be produced and distributed to all our classrooms. This new video will show teachers how to use simple manipulatives like balls, bean bags, paper plates and scarves in the classroom. Measure of objective #2 Accomplishment Our teachers will demonstrate competency and confidence by presenting 20 minutes of activities to their children on a daily basis. They will also promote and educate children on the importance of physical activity for healthy living. After each training session, teachers will continue teaching that sessions activities for the rest of the week. They will keep track of their progress with activity checklists. Objective #3 By the end of the 99/00 school year 30% - 60% of the parents and guardians whose children are involved in this program will demonstrate an increased awareness of the importance, and thereby participate in and encourage active lifestyles among their children. Steps to accomplish objective #3 1 Beginning in October 1999 the parents of the children at the four new sites as well as the eight previous sites will receive the Moving Our Children newsletter, which shares information and activities. 2. The parents of the children at the four new sites will be invited to two Family Play Time nights throughout the year, while last year's sites will receive two as well. These programs will feature healthy snacks and entertainment that promotes physical activity. 3. Parents will be Riven an opportunity to help their children win fitness related prizes for their children by participating in activities with them and keeping track on a checklist. Measure of objective #3 accomplishment The parent activity lists will indicate that some parents are encouraging and participating in physical activities with their children. The newsletters will report examples of the progress and improvements with the children during last years program, which will motivate these parents to get involved with their children's health.
