RESOLUTION
RE: APPROVE CHANGE ORDER LETTER#2 TO CHLAMYDIA TESTING CONTRACT
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#2 to the
Chlamydia Testing Contract between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department
of Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing January 1, 1999, and ending December 31, 1999, with further terms
and conditions being as stated in said change order letter, and
•
WHEREAS, after review, the Board deems it advisable to approve said change order
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#2 to Chlamydia Testing Contract between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
the Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is. authorized
to sign said change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 23rd day of June, A.D., 1999, nunc pro tunc January 1, 1999.

DATE: Change Order Letter'#2 to Chlamydia Testing Contract
Enclosed for Board review and approval is Change Order Letter#2 to the Chlamydia Testing
Contract between the Weld County Department of Public Health and Environment (WCDPHE)
and the Colorado Department of Public Health and Environment.
This change order letter provides for an additional $1,092 in funding for chlamydia tests done in
the sexually transmitted disease and family planning clinics. With this additional funding, the
total amount WCDPHE will be reimbursed for chlamydia tests for the time period January 1,
1999 through December 31, 1999 is $6,318.
I recommend your approval of this change order letter.

Contract Change Order Letter Number 2
Pursuant to paragraph 4 of the contract with contract routing number 97-06275 and contract encumbrance number
EP19706275 (as amended by Contract Amendment L contract routing number 98-07899,Contract Change Order Letter
contract routing number 99-00029,and Contract Renewal Letter 1 contract routing number 99-00087),hereinafter referred
to as the"Original Contract"(a copy of which is attached hereto and by this reference incorporated herein and made a part
hereof)between the State of Colorado,Department of Public Health and Environment and Weld County Health
Department for the term from January 1, 1999 through December 31, 1999 the parties agree that the maximum amount
payable by the State for the eligible services referenced in paragraph 2 of the Original Contract is increased by ONE
THOUSAND NINETY-TWO DOLLARS.($1,092.00)according to the budget outlined below,for a new total financial
obligation of the State of FIFTEEN THOUSAND TWO HUNDRED THIRTEEN DOLLARS,($15,213.00). The first
sentence in paragraph 2 of the Original Contract is hereby modified accordingly. All other terms and conditions of the
Original Contract are hereby reaffirmed.

This amendment to the Original Contract is intended to be effective as of July 1, 1999. However, in no event shall this
amendment be deemed valid until it shall have been approved by the State Controller or such assistant as he may
designate.
