RE: APPROVE CONTRACT AMENDMENT LETTER NO. 1 BETWEEN THE WELD
COUNTY HEALTH DEPARTMENT AND THE COLORADO DEPARTMENT OF
PUBLIC HEALTH AND ENVIRONMENT AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Amendment Letter No. 1
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department and the
Colorado Department of Public Health and Environment, with terms and conditions being as
stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Contract Amendment Letter No. 1 between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Health Department and the Colorado Department of Public Health
and Environment, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 7th day of August, A.D., 1996.

CONTRACT AMENDMENT LETTER NO. 1
In accordance with Paragraph II.C. of Contract dated January 1, 1996, routing number
9605019 contract number FAA AIR9605019 (by this reference made a part hereof) ,
between the State of Colorado Department of Public Health & Environment, Air
Pollution Control Division and Weld County Health Department, covering the period
January 1, 1996 through December 31, 1996 the undersigned agree that the maximum
amount payable by the State for the defined services is being increased by $4, 051.00
to a new total of $37, 816.44 and is to be spent in the following manner:
The State agrees to reduce the match requirement for this Contract from
$21, 931.00 to $17, 880.00 and increase the reimbursement amount for the third
quarter permit & enforcement activities by $4,051.00 to a new total of
$21, 269.00 .
Paragraphs II.8.2. and II.A.1. are hereby modified accordingly. The terms and
conditions of the original contract shall remain the same.
This amendment to the contract is intended to be effective as of 07/01/96, but in
no event shall it be deemed valid until it shall have been approved by the State
Controller or his authorized designee.

Subject: Amendment to 1996 Air Quality Control ontract
Enclosed for Board approval is Amendment Letter#1 to the 1996 Air Quality Control Contract
between Weld County Health Department and the Colorado Department of Public Health and
Environment (CDPHE).
The Air Pollution Control Division of the State Health Department has recently obtained
additional funds for the local agencies. The amount designated to WCHD is $4,051 and will be
used to reduce the local match requirement for this contract from $21,931 to $17,880. The
reimbursement amount for the third quarter permit and enforcement activities will be increased
by $4,051.
This amendment will bring the maximum amount reimbursable by CDPHE for the Air Quality
Control Contract to $37,816.44 for the period January 1, 1996 through December 31, 1996.
I recommend your approval of this amendment.
