RE: APPROVE CONTRACT RENEWAL LETTER NO. 1 FOR CHLAMYDIA TESTING
BETWEEN THE WELD COUNTY HEALTH DEPARTMENT AND THE COLORADO
DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter No. 1 for
Chlamydia Testing between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, commencing January 1, 1995, and ending
December 31, 1995, with further terms and conditions being as stated in said renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract Renewal Letter No. 1 for Chlamydia Testing between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Health Department, and the Colorado Department of Public
Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 18th day of January, A.D., 1995, nunc pro tunc January 1, 1995.

Contract Renewal Letter No. 1
In accordance with Paragraph 3 of contract routing number 94-0876 of contract number
EP1940876 (copy attached and by this reference made a part hereof as Attachment c ) between
the State of Colorado Department of Public Health and Environment (Disease Control and
Environmental Epidemiology Division, Communicable Distance Section) and Weld County
Health Department covering the period of January 1, 1995 through December 31, 1995 the
undersigned agree that the maximum amount payable by the State for eligible chlamydia testing
services is$3,113 to be spent according to the budget outlined below. The terms and conditions
of the original contract shall remain the same.
Family Planning Clinic chlamydia tests
(195 @ $7.50 each) $ 1,463
STD Clinic chlamydia tests
(women and male contacts)
(220 @ $7.50 each) $ 1,650
TOTAL $ 3,113
This amendment to the contract is intended to be effective as of 01/01/95, but in no event shall
it be deemed valid until it shall have been approved by the State Controller or such assistant as
he.m.y designate.

Renewal Letter for Chlamvdia Testing Contract
Attached for Board review and approval is a renewal letter for the Chlamydia Testing contract
between the Weld County Health Department and the Colorado Department of Public Health and
Environment.
This renewal letter provides for a combined total of 415 chlamydia tests for Family Planning
Clinic and STD Clinic clients during the period January 1, 1995 through December 31, 1995.
Weld County will be reimbursed $7.50 per test up to a maximum of $3,113. This is a decrease
of 245 tests and $1,837 from last calendar year's contract.
I recommend your approval of this renewal letter.
