(ACI) would like to provide you with construction surveying on the above referenced site. We are especially qualified to provide those services for the following reasons: I. Accurate is experienced in this type of work and has completed similar jobs in the past. 2. We have the available personnel to provide prompt and efficient service. We currently dispatch 11 construction-oriented field crews from our Broomfield and Colorado Springs offices. 3. ACI uses only state-of-technology field and office equipment. 4. We have a workload classified as medium, so new projects can be scheduled. Our familiarity with this type of project enables us to provide the following construction surveying services on a fixed fee basis per the attached rate schedule. If any items are not required, please line them out of the proposal. This proposal is based on plans designed by Sky Associates and provided by Rhoads Construction. Scope of Services $ite Improvements Curb and Gutter (606): Provide one set of stakes for construction of the $1 ,550 curb and gutter and asphalt edges in the parking lot_ Stakes shall be set on a predetermined offset from the top back of curb (TBC)or edge of asphalt(EA) on intervals of 25 feet and at P.C's and P.T's of curves,curved sections and angle points. Grades shall be marked to TBC or EA. Grade sheets shall be provided. For this project 1253 L.F. are estimated. Blue Tops (613): Provide one set of stakes for fine grading. Stakes shall $ 350 be set to finished elevation of gravel grade. Stakes shall be set on a 50 foot grid and protected by a guard stake. For this task 8 points are estimated. * * * * * * * * * * * * The following items arc specifically excluded from this proposal. If sufficient time is given, fees for these items will be provided prior to start of services. 1. Project meetings, except as noted. 2. Boundary retracement. 3. Demolition lines. 4. Topography checks and calculations of quantities for site balance or removals. 5. Record drawings, except as noted. 6. Restaking. 7. Landscaping. • $. Prints, delivery services and other reimbursable expenses will be billed at cost plus 10%, and arc not a part of this agreement. The following insurance is included in our rates: * General Liability Insurance ($1,(X0,(XX0 per occurrence / $2,000,000 aggregate) * Automobile Liability Insurance ($1,(X)0,000 per occurrence) * Umbrella Insurance ($1,000,000) * Workers Compensation Insurance (statutory) * Professional Liability Insurance ($1,OW,000 per occurrence / $1,(XX),0(10 aggregate) Insurance in addition to the above will require an increase in our fees. The client agrees to pay Surveyor compensation for services performed on a fixed fee basis except as noted. The fee., quoted herein shall be used as a guide in determining the percentage of work completed by the Surveyor. Progress billings shall be made towards the end of each month for services performed during that period. All bills arc due and payable upon receipt of invoice. EXTRA STAKING: If restaking or additional services are required after the Surveyor has set the proposed sakes as indicated in the proposal, the client will assume the cent of said restaking. Any required restaking or additional services must be authorized in writing by the client's designated representative at the time the services are requested. Service designated as extra will he billed at Surveyor's hourly rates per the prevailing Accurate Consultant's, Inc. rate schedule. NOTIFICATION: It is understood whenever the contractor on the job needs survey slaking services performed, they will make an attempt to schedule such services in S-hour increments in order to reduce unnecessary travel time. They will also attempt to provide the Surveyor with a minimum of 24-hour notice before such service is to be performed. RETENTION; Costs shown in scope of services arc based on progress payments being paid in full when invoiced with no retention.
