RE: APPROVE SIX REVISIONS TO NOTIFICATIONS OF FINANCIAL ASSISTANCE
AWARDS FOR PAC FUNDS WITH VARIOUS ENTITIES AND AUTHORIZE CHAIR TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with six Revisions to Notifications of
Financial Assistance Awards for PAC (Placement Alternatives Commission) Funds between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Social Services, and the following entities:
1. Island Grove Regional Treatment Center, Inc.
Individualized Family Systems Sexual Abuse Treatment
2. First Steps of Weld County, Inc.
Life Skills - Teen Pregnancy Program
3. Weld Mental Health Center, Inc.
Carson Children's Center Day Treatment Program
4. Weld County Department of Social Services
Generic Day Treatment
5. Weld County Department of Social Services
Generic Sexual Abuse Treatment
6. Alternative Homes for Youth
Greeley Day Treatment
WHEREAS, said revisions to notifications commence June 1, 1996, and end May 31,
1997, with further terms and conditions being as stated in said revisions to notifications, and
WHEREAS, after review, the Board deems it advisable to approve said revisions to
notifications, copies of which are attached hereto and incorporated herein by reference.

NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the six Revisions to
Notifications of Financial Assistance Awards for PAC Funds between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Department of Social Services, and the above-named entities be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said revisions to notifications.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 10th day of July, A.D., 1996, nunc pro tunc June 1, 1996.

SUBJECT: Revisions to FY96-97 Notification of Financial Assistance Awards (NOFAAs)
Enclosed for Board approval are revisions to FY96-97 Notification of Financial Assistance Awards
(NOFAAs) for Placement Alternatives Commission (PAC) Funds, which are for the period of June 1,
1996 through May 31,1997.
These revisions do not affect the approved total funding of each agency. These revisions clarify or
correct hourly\monthly rates provided to these agencies as follows:
1. Island Grove Regional Treatment Center
Sexual Abuse Treatment:: The one hourly rate average has been changed to reflect different
hourly rates for various types of service.
2. First Steps of Weld County. Inc.
Life Skills- Teen Pregnancy Program: The one hourly rate average has been changed to reflect
different hourly rates for various types of service.
3. Weld Mental Health Center. Inc.
Day Treatment Program - Carson Children's Center: The monthly rate has been corrected to
$1,250.
