RE: APPROVE CHANGE ORDER LETTER #2 FOR HIV PREVENTION CONTRACT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#2 for the HIV
Prevention Contract between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing January 1, 1999, and ending December 31, 1999, with further terms and
conditions being as stated in said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#2 for the HIV Prevention Contract between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
the Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 6th day of October, A.D., 1999, nunc pro tunc January 1, 1999.

September 14, 1999 Contract Change Order Letter
State Fiscal Year 2000-01, Contract Change Order Letter Number 2, Contract Routing Number 00-00088
Pursuant to paragraph 4 of Section E of the contract with contract routing number 99-00106 and contract encumbrance
number PO FHA SET9900106 (as amended by Contract Change Order Letter Number 1,contract routing number 00 FHA
00064),hereinafter referred to as the"Original Contract"(a copy of which is attached hereto and by this reference
incorporated herein and made a part hereof)between the State of Colorado,Department of Public Health and
Environment and Weld County Board of Commissioners for the use and benefit of Weld County Health Department
for the term from January 1, 1999 through December 31, 1999 the parties agree that the maximum amount payable by the
State for the eligible services referenced in paragraph 1 of Section E of the Original Contract is incre HREE �}
THOUSAND DOLLARS ($3,000.00)for a new total financial obligation of the State of TWENTY THOUSAND
SIX HUNDRED NINETY DOLLARS ($27,690.00). The first sentence in paragraph 1 of Section E of the Original
Contract is hereby modified accordingly. All other terms and conditions of the Original Contract are hereby reaffirmed.
This amendment to the Original Contract is intended to be effective as of October 15,1999. However,in no event shall this
amendment be deemed valid until it shall have been approved by the State Controller or such assistant as he may designate.
Please sign, date,and return all 4 originals of this Change Order Letter by September 30, 1999 to the attention of:Jennifer

SUBJECT: Change Order Letter#2 to the HIV Prevention Contract
Enclosed for Board review and approval is Change Order Letter#2 to the HIV Prevention
Contract between the Colorado Department of Public Health and Environment and Weld County
Department of Public Health and Environment (WCDPHE).
The purpose of this change order letter is to provide additional funding for the Coloradans
Working Together Coalition who target HIV education and prevention in northeastern Colorado.
This letter will add an amount not to exceed $3,000 which will bring the total funding of the
contract to $27,690 for the time period January 1, 1999 through December 31, 1999.
I recommend your approval of this change order letter.
