RE: APPROVE MEMORANDUM OF AGREEMENT FOR PEER INTERVENTION AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Memorandum of Agreement for Peer
Intervention between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Health Department, and the Colorado
Department of Public Health and Environment, commencing November 3, 1997, and ending
June 30, 1998, with further terms and conditions being as stated in said memorandum of
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said memorandum of
agreement, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Memorandum of Agreement for Peer Intervention between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Health Department, and the Colorado Department of Public
Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said memorandum of agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 10th day of December, A.D., 1997, nunc pro tunc November 3,
1997.

Memorandum of Agreement
Between the Colorado Department of Public Health and Environment,Women's Health Section
and
Weld Count Health Department
For the Period November 3, 1997 through June 30, 1998
This document details the services to be provided by Weld County Health Department as part of the Peer
Intervention Projects to Reduce Unintended Pregnancy. Weld County Health Department has been
awarded $8,500 to develop a peer educator/mentor teen outreach program and to provided a teen
health clinic. Four peers will be trained the first year and a teen clinic that offers basic health
services including family planning methods will be offered twice a week.
Records will be kept on all student presentations, including attendance, and participant evaluations. The
number of patients served in the teen clinic will be tracked. Evaluation of this project is vital;
therefore,the Women's Health Section has contracted with Kaia Gallagher,Ph.D. and her colleagues to
provide technical assistance on data collection and evaluation. The agency will contact Dr. Gallagher at
(303)315-9700 for assistance in finalizing an evaluation plan to be submitted with the midpoint review.
The consultation will be paid for by the Women's Health Section.
Funding for the project begins November 3, 1997 and ends June 30, 1998. Billing will be done each
month for one-eight of the amount($1,062.50)beginning November 30. A midpoint project update is
due February 27, 1998 and a final report and evaluation is due August 15, 1998.

Peer Intervention Memorandum of Agreement
Enclosed for Board review and approval is Memorandum of Agreement between the Colorado
Department of Public Health and Environment (CDPHE) and Weld County Health Department
(WCHD) for the Peer Intervention Program.
The CDPHE has awarded WCHD $8,500 for the implementation of the Greeley Teen Pregnancy
Prevention Outreach Program. The purpose of this program is to promote family planning
services, build negotiation and refusal skills, prevent premature sexual activity, prevent sexually
transmitted diseases, and to ultimately reduce the incidence of unintended teen pregnancies. This
program will work in conjunction with our family planning outreach clinic at the Monfort
Children's clinic and will include hiring peer educators to provide educational programs and
support for the clients.
Funding for this project is for the time period beginning November 3, 1997 and ending June 30,
1998.
