BASIS OF CLAIM: Claimant' s claim against the County of Weld, Colorado, is based upon a traffic incident which occurred on January 27 , 1996 , at the location indicated on the attached accident report due to the failure of the county to properly sand and maintain such intersection. The Claimant Celia R. Panowicz sustained injuries, damages, and losses as a result of the negligence of the employees of the County of Weld. The incident and resulting injuries, damages, and losses resulted from the acts and/or omissions on the part of the employees of the County of Weld in that said employees failed to provide proper grading, sanding, and repair of the intersection . NAME OF PUBLIC EMPLOYEES : The driver/operator of the aforementioned Weld County Road maintenance vehicle is unknown at this time as are other individuals responsible for the maintenance of said intersection and the supervision of the safety provided to the general public. NATURE AND EXTENT OF INJURY AND DAMAGE: The full nature and extent of injuries and damages suffered by the Claimants are not fully known at this time, but such injuries and damages include, but are not limited to the following: 1 . Fractured left arm, neck and back injuries; 2 . Ambulance/EMT expenses; 3 . Medical doctor, hospital, and rehabilitation expenses; 4 . Scrapes, bruises and possible disfigurement; 5 . Loss of enjoyment of life; 6 . Pain and suffering; 7 . Emotional distress. 2 MONETARY DAMAGES CLAIMED: The full extent of monetary damages has not yet been fully ascertained. It appears that damages will probably exceed $100,000 . 00 .
