Dear Applicant:
This letter is to inform you that your dance license expired December 31, 1996. Enclosed is another
application form for your 1997 dance license,which should be completed and returned with a$25.00
fee. Please sign the application form, and our office staff will notarize if you are unable to obtain
a notary. Your application will be considered for approval by the Board of County Commissioners
at a regular board meeting. Upon approval, a new license will be issued and mailed to you.
Your check should be made payable to the Weld County Clerk to the Board and remitted to the
above address as soon as possible. Please be advised this is the last letter you will receive from our
office informing you that your 1996 dance license has expired.
