RE: APPROVE FOUR AMENDMENTS TO NOTIFICATION OF FINANCIAL ASSISTANCE
AWARDS AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Four Amendments to Notification of
Financial Assistance Awards between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and North Range Behavioral Health, commencing June 1, 1997, and ending May 31,
1998, with further terms and conditions being as stated in said amendments, and
WHEREAS, after review, the Board deems it advisable to approve said amendments, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Four Amendments to
Notification of Financial Assistance Awards between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Department
of Social Services, and North Range Behavioral Health be, and hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendments.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 8th day of October, A.D., 1997.

RE: Amendments to Notification of Financial A sistance '
Awards (NOFAs) with North Range Behavioral Health
under the Placement Alternatives Commission Funds
Enclosed for Board approval are amendments to Notification of Financial Assistance Awards
(NOFAs) with North Range Behavioral Health under the Placement Alternatives Commission
(PAC) funds.
These NOFAs are necessary to reflect that service to Social Services clients who are Medicaid
eligible will not be reimbursed out of PAC Core Service Funds. These services are to be paid out
of Medicaid funds provided to North Range Behavioral Health. The Weld County Department
of Social Services will continue to pay for non-Medicaid eligible clients. All other conditions for
the programs will remain the same.
The NOFAs affect the PAC programs authorized for North Range Behavioral Health as follows:
1. Carson Children's Center Day Treatment Program
2. Intensive Family Therapy Program
3. Sexual Abuse Treatment
4. Option B - Mobile Mental Health Team
