RE: APPROVE CONTRACT RENEWAL LETTER#1 TO PRENATAL PROGRAM
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter#1 to the
Prenatal Program Contract between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Health Department, and the
Colorado Department of Public Health and Environment, commencing October 1, 1998, and
ending September 30, 1999, with further terms and conditions being as stated in said renewal
letter, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract Renewal Letter#1 to the Prenatal Program Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Health Department, and the Colorado
Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 21st day of October, A.D., 1998, nunc pro tunc October 1, 1998.

Contract Renewal Letter#1 to the Prenatal Program Contract
Enclosed for Board review and approval is Contract Renewal Letter#1 to the Prenatal Program
Contract between the Colorado Department of Public Health and Environment and Weld County
Health Department.
Under the provisions of this agreement, Weld County Health Department will provide prenatal
and postpartum care, including education and counseling, for 150 low income women and
Prenatal Plus enhanced service for 55 women during the period October 1, 1998 through
September 30, 1999. You will recall the Health Department is entering into an agreement with
North Colorado Family Medicine for that agency to provide part of these services. The renewal
letter provides for Weld County to be reimbursed $137,500 which is $10,500 more than last
year's agreement.
I recommend your approval of this contract renewal letter.

Dear Linda,
Find here the renewal letter for your prenatal contract. As we
discussed I have changed the language in the renewal to say your
agency will "assure" rather than provide services.
If this change meets with John' s approval, please have him sign and
return all copies of this letter to me for processing. When all state
approvals have been given, I will return a fully executed copy for
your agency's files.
If this change is not okay with John, please call me immediately so
that we can discuss the terms of your October 1, 1998 through
September 30, 1999 Prenatal Contract.

Contract Renewal Letter No. 1
In accordance with Paragraph 13 of the contract with routing number 9705717 and contract number
PRN9705717, as renewed with contract routing number 9807729, hereinafter referred to as the Original
Contract(copy attached and by this reference made a part hereof) between the state of Colorado, Department
of Public Health and Environment, Prenatal Program and Weld County Health Department,the parties hereby
by agree the Original Contract is hereby renewed for the period October I, 1998 through September 30, 1999.
The parties agree that the maximum amount payable by the State for the eligible services during this renewal
period is One hundred thirty-seven thousand two hundred fifty Dollars ($137.2501. The state will pay Sixty
seven thousand five hundred Dollars ($67.500)to administer the program, purchase medical supplies and
pharmaceuticals and to assure medical, laboratory, education and counseling services for 150 low income
women, Forty five thousand Dollars($45.000 1 for diagnostic tests and,Twenty four thousand seven hundred
fifty Dollars($24,7501 for Prenatal Plus Enhanced services to 55 women. The terms of conditions of the
original contract are hereby reaffirmed and shall continue in full force and effect throughout this renewal
period unless otherwise stated.
This amendment to the Original Contract is intended to be effective October 1, 1998, but in no event shall it
be deemed valid until it shall have been approved by the State Controller or such assistant as he may
designate.
Please sign, date and return all originals of this letter as soon as possible to:
