RE: APPROVE LICENSE AGREEMENT FOR USE OF ENVIRONMENT, HEALTH AND
SAFETY HOMEPAGE AND AUTHORIZE CHAIR TO SIGN -WEBSITE SAFETY
SYSTEMS, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a License Agreement for use of the
Environmental, Health and Safety Homepage between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the Weld
County Sheriffs Office, and Website Safety Systems, Inc., commencing December 1, 1999,
and ending December 31, 2001, with further terms and conditions being as stated in said
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the License Agreement for use of the Environmental, Health and
Safety Homepage between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Sheriffs Office, and
Website Safety Systems, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of November, A.D., 1999.

LICENSE AGREEMENT
PROGRAM: ENVIRONMENTAL, HEALTH & SAFETY HOMEPAGE
WEBSITE SAFETY SYSTEMS, INC
This agreement is made between WEBSITE SAFETY SYSTEMS, INC., a corporation in the
state of Pennsylvania, and the LICENSEE named below.
1. General- WEBSITE SAFETY SYSTEMS, INC. (WSSI) hereby grants to LICENSEE a
non-exclusive, non-transferable right to use the Environmental, Health & Safety Homepage
(EHS Homepage) at one facility, subject to the terms and conditions set forth below.

3. Limitations of the License and Restrictions:
a. LICENSEE agrees that it shall use the EHS Homepage only for the LICENSEE'S
sole and exclusive use, and shall not disclose, sell, license, or otherwise
distribute the program to any third party without the prior written consent of
WSSI.
b. LICENSEE agrees to use the EHS Homepage at one facility only, unless
authorized in writing by WSSI to use the EHS Homepage at multiple facilities.
c. License is valid from December 1, 1999 to December 31, 2001.
4. Disclaimer of Warranty and Limitation of Liability:
The EHS Homepage is provided "As Is"without warranty of any kind, either expressed or
implied, including without limitation the implied warranties of merchantability and fitness
for a particular purpose. WSSI does not warrant that the information contained in the
Homepage will meet LICENSEE'S requirements or that the operation will be
uninterrupted or error free. WSSI disclaims any liability or responsibility for the violation
of any federal, state or local regulations with respect to any claim on account of or
arising from this Agreement or use of this EHS Homepage.
5. Considerations:
LICENSEE agrees to pay WSSI an annual fee of$450.00. Payment shall be made
to "Website Safety Systems, Inc.", 270 Mather Road, Jenkintown, PA, 10946-3129.
Federal EIN-23-2851403. Net payment due 30 days after the "License is valid from"
date (section 3.c. above).

Dear Larry,
Lets try this again. The website is a very economical way to keep up to date
with MSDSs and written safety programs.Your site will be customized with your
laboratory's name. A logo can also be added.
Enclosed is the annual license agreement for the use of the Environmental,
Health, Safety Homepage. There are two copies. Please keep one for your
records and return one with your payment. Payment can be made by check.
Please send purchase orders to the attention of Mr. David Ridyard. If a invoice
needs to be sent back, please include the name and address of who to send it to
The license fee is $450.00 a year. This is for a single facility license and can
only be used at one facility. This rate is for ASCLD members only and part of
the fee goes to support ASCLD. The agreement is dated for December 1, 1999
and it is valid to December 31, 2001.
