RE: APPROVE CHANGE ORDER LETTER#2 TO IMMUNIZATION ACTION PLAN
CONTRACT WITH THE COLORADO DEPARTMENT OF PUBLIC HEALTH AND
ENVIRONMENT AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#2 to the
Immunization Action Plan Contract between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Health Department, and the Colorado Department of Public Health and Environment,
commencing July 1, 1997, and ending December 31, 1998, with further terms and conditions
being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#2 to the Immunization Action Plan Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 28th day of July, A.D., 1997, nunc pro tunc July 1, 1997.

Change Order Letter#2 to the Immunization Action Plan Contract
Enclosed for Board approval is Change Order Letter#2 to the Immunization Action Plan
Contract between Weld County Health Department and the Colorado Department of
Public Health and Environment.
This letter will provide an additional sum up to $20,000 to be used to increase the
immunization levels for children aged birth to two years of age in Weld County. This
funding will be used to employ an additional full-time temporary office technician who
will provide data entry for the immunization tracking system and also pay for equipment
and supplies to make immunization photo ID cards. These cards are given to the parents
of newborns to remind them to have their child immunized.
The term of this Change Order Letter is from July 1, 1997 through December 31, 1998.
This program continues to focus on a critical public health population, and I recommend
your approval.
