RE: Certification of Expenses In-
curred in Emergency Vector
Control Program .
roe Colorado Disaster Office and the Colorado Department of Agriculture
have established the procedure for making claims, obtaining advance of
funds and final audit for emergency expenses incurred by the disaster
counties for vector control.
Procedures are as follows:
(1) Eligible counties will certify to the Colorado State De-
partment of Public Health the total of expenses for emer-
gency vector control for which claim for reimbursement is
being made, This should include costs incurred by cities
and towns over and above regularly budgeted funds for this
purpose. It is suggested that the form of certificate be
similar to the attached. Certification should include
only those costs eligible for reimbursement as outlined
in. 0.E.P. Circular 4000.5A
(2) me Colorado State Department of Public. Health will
certify the necessity of the expense to the Colorado De-
partment of Agriculture.

The Colorado Department of Agriculture will advance 75%
of the claim to the County from the funds received from
0.E.P.
Final. payment on the claim will be made after an. audit
of each county's claim by the Disaster Office. It is
suggested that all vouchers, invoices, warrants covering
emergency vector control be combined in one file for easy
access and inspections for final audit. This will assist
the auditor and speed up final payment,
If no reimbursible expenses were incurred, please submit a negative report
so we may clear the records. All certified claims presented to this De-
partment will be processed immediately.

SUGGESTE➢ LETTER OF CERTIFICATION
It is hereby certified that County and its incorporated
subdivisions or deputized agencies incurred the expenses in the amount
of $ for the emergency vector control during the emer-
gency period following 16 June 1965.
Application is hereby being made for advance of seventy-five per
cent of the encumbered amount, with the understanding that final payment
will be received after audit of the paid vouchers or warrants by the re-
presentatives of the Office of Emergency Planning or Colorado Disaster
Office, subject to the eligibilities established in O,E.P. Circular
4000.5A.
