RE: APPROVE CHANGE ORDER LETTER #1 FOR SINGLE ENTRY POINT CONTRACT
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#1 for Single
Entry Point Contract between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services,
Area Agency on Aging, and the State of Colorado, Department of Health Care Policy and
Financing, with terms and conditions being as stated in said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#1 for Single Entry Point Contract between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and
State of Colorado, Department of Health Care Policy and Financing, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of April, A.D., 1998.

Change Order Letter No. 1
In accordance with Paragraph 10 of Page 2 of Exhibit A of contract routing number 3398-1323 between the
State of Colorado Department of Health Care Policy and Financing and Weld County Board of
Commissioners by and through the Weld County Division of Human Services Area Agency on Aging, 1551
North 17th Avenue, Greeley, CO 80631 covering the period of July 1, 1997 through June 30, 1999 the
undersigned agree the first sentence of Paragraph 9, Page 7, Compensation/Maximum Payable is hereby
amended to read:
Payment pursuant to this Contract will be made as earned, in whole or in part, from available
state funds encumbered in an amount not to exceed $294,668 for the purpose of the within
described services.
This constitutes a(n) decrease in the maximum compensation to be made to the Contractor by the State
under this contract of$14,652.00.
The sentences on Page 2 of Exhibit A to the contract which now read:
However, total compensation for the contract term shall not be increased under this
mechanism by more than $33,374.00 above the base contract amount, i.e., to a maximum
total compensation of$309,320.00. The maximum increase above the base contract
amount represents 41 clients above the base client figure; the figure of 380 shall be known
as the "maximum reimbursable client figure".

compensation of$294,668. The maximum increase above the base contract amount
represents 23 clients above the base client figure; the figure of 362 shall be known as the
"maximum reimbursable client figure".
This amendment to the contract is intended to be effective as of April 13, 1998, but in no event shall it be
deemed valid until it shall have been approved by the State Controller or such assistant as he may
designate.

Attached is an amendment to the original Single Entry Point Contract. The amendment to the Single
Entry Point Contract provides information regarding the maximum payment that will be received for
Fiscal Year 1997-1998. The amount will not exceed $294,668.00.
