RE: APPROVE CONTRACT AMENDMENT FOR JUVENILE DIVERSION PROGRAM AND
AUTHORIZE CHAIR TO SIGN - THE COLORADO DEPARTMENT OF PUBLIC
SAFETY, DIVISION OF CRIMINAL JUSTICE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS.. the Board has been presented with a Contract Amendment for the Juvenile
Diversion Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the 19th Judicial District Attorney's
Office, and the Colorado Department of Public Safety, Division of Criminal Justice, with terms
and conditions being as stated in said amendment, and
WHEREAS. after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract Amendment for the Juvenile Diversion Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the 19th Judicial District Attorney's Office, and the
Colorado Department of Public Safety, Division of Criminal Justice, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of April, A.D., 1999.

CONTRACT AMENDMENT #
THIS AMENDMENT, made this 16th day of April 1999, by and between the State of
Colorado for the use and benefit of the Department of Public Safety, Division of Criminal Justice,
700 Kipling Street, Suite 1000, Denver, Colorado, 80215 hereinafter referred to as the State, and
Office of the District Attorney, 19th Judicial District, P. O. Box 1167, Greeley, Colorado 80634
hereinafter referred to as the contractor,
FACTUAL RECITALS
Authority exists in the Law and Funds have been budgeted, appropriated, and otherwise
made available and a sufficient unencumbered balance thereof remains available for payment in
Fund Number 100, Appropriation Code 764 , Contract Encumbrance Number 99000000018;
and
Required approval, clearance, and coordination has been accomplished from and with
appropriate agencies; and
The Colorado Department of Public Safety, Division of Criminal Justice, has funds that
may be utilized within the scope of juvenile diversion programs, as defined in the "Colorado
Children's Code," for Fiscal Year 1998-99;
The Contractor has submitted a proposal to provide additional restitution services to youth,
or to purchase needed supplies or equipment in order for the contractor to better serve youth in
the 19th judicial district.
NOW THEREFORE, it is hereby agreed that
1. Consideration for this Amendment to the Original Contract Number 99000000018, dated
July 1, 1998, consists of the payment which shall be made pursuant to this Amendment and the
promises and agreements herein set forth.
2. It is expressly agreed by the parties that this Amendment is supplemental to the Original
Contract Number 99000000018, dated July 1, 1998, which is, by this reference, incorporated,
made a part hereof, and identified as Exhibit "A," and all terms, conditions, and provisions
thereof, unless specifically modified herein, are to apply to this Amendment as though they were
expressly rewritten, incorporated, and included herein.
3. It is agreed. the Original Contract is and shall be modified, altered, and changed in the
following respects only:

a. Increase original encumbrance of$49,829 by $294 for a total contract amount of
$50,123.
b. Funds to be used to purchase office equipment for the Juvenile Diversion program
as specified in the proposal dated April 1, 1999.
4. The effective date of this Amendment is May 15, 1999.
5. Except for the "Special Provisions," in the event of any conflict, inconsistency, variance,
or contradiction between the provisions of this Amendment or any of its attachments or exhibits,
and any of the provisions of this Amendment shall in all respects supersede, govern, and control.
The "Special Provisions" shall always be controlling over other provisions in the contract or
amendments. The factual representations in the Special Provisions concerning the absence of
bribery or corrupt influences and personal interest of State employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT
FISCAL YEAR ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING
APPROPRIATED, BUDGETED, AND OTHERWISE MADE AVAILABLE.
7. THIS AMENDMENT SHALL NOT BE DEEMED VALID UNTIL IT SHALL HAVE
BEEN APPROVED BY THE CONTROLLER OF THE STATE OF COLORADO OR SUCH
ASSISTANT AS HE MAY DESIGNATE.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the day
first above written.

RE: Additional Funding that is Available for Diversion Programs
Dear Mindy:
This is a request for the 19th Judicial District Attorney's Office: Juvenile Diversion Program to
apply for items to be purchased due to the additional funds that may be available through DCJ as
unallocated funds.
NARRATIVE DIESCRIPTION
The Juvenile Diversion Program receives limited funds from Weld County for operating and
supply expenses each year. The items that I have listed below in our priority list are prioritized
according to need. These items would be purchased from Brattons and Signs Now. Bretton is
an office supply store located in Greeley and Signs Now makes a variety of different signs. I
will list order numbers as reference for the Brattons supplies and I will list the estimate given to
me by Signs Now for the work they would be completing for Diversion.
1. Tufide Vinyl Catalog Case, Black $126.00

Need: This case will be used by our Fort Lupton Juvenile Diversion Counselor. He is in
Fort Lupton two days a week and utilizes this case to carry all his files and other needed
materials back and forth between Greeley and Fort Lupton. The case he is currently
using needs to be replaced. •
2. Vinyled Foam 3MM PVC Table Sign $52.50 -1k-
PVC-3MM, Multi on White 17x12
Need: This sign will be used by staff at various college or high student fairs where
Juvenile Diversion/Victim Assistance has an information table. The sign will sit on the
top of the table.

Need; These computer workstation tables are for two of the Diversion counselors. These
tables will allow for their PC computer/keyboards to be moved off their desks to allow
for more workspace and also help to make the use of the computer more ergonomically
correct.

Necdt This chair is for our Community Programs Administrator's office. This will allow
her to have a chair with wheels and also a more ergonmically correct chair to work at her
separate computer table.
