RE: APPROVE CANCER PREVENTION OUTREACH CONTRACT AMENDMENT #1
BETWEEN HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with the Cancer Prevention Outreach
Contract Amendment #1 between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, commencing
July 15, 1997, and ending August 15, 1997, with further terms and conditions being as stated in
said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Cancer Prevention Outreach Contract Amendment #1 between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the Colorado Department
of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 5th day of May, A.D., 1997.

CONTRACT AMENDMENT #1
THIS AMENDMENT, made this 11th day of April, 1997, by and between the State of Colorado for the use and
benefit of the Department of Public Health and Environment, 4300 Cherry Creek Drive South, Denver, Colorado
80222-1530, hereinafter referred to as the State, and Weld County Health Department. 1517 16th Avenue Court
Greeley. CO 80631 hereinafter referred to as the contractor.
WHEREAS, authority exists in the Law and Funds have been budgeted, appropriated, and otherwise made available
and a sufficient unencumbered balance thereof remains available for payment in Fund Number 100, Appropriation
Code 773, Contract Encumbrance Number PPG9706161; and
WHEREAS, required approval, clearance, and coordination has been accomplished from and with appropriate
agencies; and
WHEREAS, the parties entered into a contract dated November 15, 1996, contract number PPG9706161, routing
number 9706161 ("Original Contract"), whereby contractor was to provide certain services to the State; and
WHEREAS, the Original Contract is funded from a cooperative agreement between the state and the Centers for
Disease Control & Prevention (CDC) and the CDC has extended the end date of the cooperative agreement from July
15, 1997, to August 14, 1997; and
WHEREAS, the State and the contractor have mutually determined that due to the extension of the cooperative
agreement term by the CDC, a one (1) month extension in the Original Contract is necessary so that there is no lapse
in community outreach services provided by contractor; and
WHEREAS, it is the State's intent to increase the contract by One Thousand Five Hundred Fifty -Eight Dollars
($1,558) to reimburse the contractor for community outreach services during the one month extension.
NOW THEREFORE, it is hereby agreed that:
1. Consideration for this Amendment to the Original Contract, contract encumbrance number PPG9706161,
Contract Routing Number 9706161, dated November 15, 1996, consists of payments which shall be made pursuant to
the original contract and the parties' promises and agreements herein set forth.
2. It is expressly agreed by the parties that this Amendment is supplemental to the Original Contract, contract
number PPG9706161, dated November 15, 1996, which is, by this reference, incorporated herein and made a part
hereof, and is attached hereto as Exhibit ONE. All terms, conditions, and provisions of the Original Contract, unless
specifically modified herein, are to apply to this Amendment as though they were expressly rewritten, incorporated,
and included herein.
3. It is agreed the Original Contract is and shall be modified, altered, and changed in the following
respects only:
a. The contract dollar amount stated on page 2, in paragraph 3 is hereby modified to ELEVEN
THOUSAND EIGHT HUNDRED THIRTY-SEVEN DOLLARS ($11,837).

c. The term of the contract, stated on page 2, in paragraph 5, is hereby modified and extended to August
14, 1997.
4. The effective date of this Amendment is June 30, 1997.
5. Except for the "General Provisions" and "Special Provisions", in the event of any conflict, inconsistency,
variance, or contradiction between the provisions of this Amendment or any of its attachments or exhibits, and any of
the provisions of the Original Contract, the provisions of this Amendment shall in all respects supersede, govern, and
control. The "Special Provisions" shall always be controlling over other provisions in the contract or amendments.
The factual representations in the Special Provisions concerning the absence of bribery or corrupt influences and
personal interest of State employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT FISCAL YEAR ARE
CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING APPROPRIATED, BUDGETED, AND
OTHERWISE MADE AVAILABLE.
7. THIS AMENDMENT SHALL NOT BE DEEMED VALID UNTIL IT SHALL HAVE BEEN APPROVED
BY THE CONTROLLER OF THE STATE OF COLORADO OR SUCH ASSISTANT AS HE MAY DESIGNATE.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the day first above written.

Amendment to the Cancer Prevention Outreach Contract
Enclosed for Board approval is Amendment #1 to the Cancer Prevention Outreach
Contract between Weld County Health Department and the Colorado Department of
Public Health and Environment (CDPHE). This contract amendment is necessary
because the CDPHE received a one -month extension from the Centers for Disease
Control.
This amendment will allow the Health Department to provide outreach activities to
recruit low income women age 50 and over for breast and cervical cancer screening
services for one more month. The current contract was scheduled to end July 15, 1997
and this will extend it until August 15, 1997. The Health Department will receive an
amount not to exceed $1,558 for this extra month of activities.
I recommend your approval of this amendment.
