RE: APPROVE AMENDMENT TO AGREEMENT FOR USE AND OPERATION OF "A
KID'S PLACE" WITH CITY OF GREELEY, COLORADO, CITY OF EVANS,
COLORADO, AND UNITED WAY OF WELD COUNTY AND AUTHORIZE CHAIR TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Agreement for Use
and Operation of"A Kid's Place" among the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Sheriff's
Office, the Weld County Department of Social Services, and the District Attorney for the 19th
Judicial District, and United Way of Weld County, the City of Greeley, Colorado, and the City of
Evans, Colorado, with terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to Agreement for Use and Operation of"A Kid's
Place" among the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Sheriff's Office, the Weld County
Department of Social Services, and the District Attorney for the 19th Judicial District, and United
Way of Weld County, the City of Greeley, Colorado, and the City of Evans, Colorado, be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 15th day of July, A.D., 1996.

AMENDMENT TO AGREEMENT FOR USE AND OPERATION OF "A KID'S PLACE"
/ THIS AMENDMENT TO AGREEMENT is made and entered into this /54iday of
( p , 1996 by and between the County of Weld, Colorado,by and through the Board
of dunty commissioners of the County of Weld, on behalf of the Weld County Sheriffs Office,
and sitting as the Board of Social Services for the Weld County Department of Social Services,
whose address is 915 Tenth Street, Greeley, Colorado 80631, hereinafter collectively referred to as
"County"; United Way of Weld County, whose address is 814 Ninth Street, Greeley, Colorado
80631, hereinafter referred to as "United Way"; the District Attorney for the 19th Judicial District,
whose address is 915 10th Street, Greeley, Colorado 80631, hereinafter referred to as "District
Attorney"; the City of Greeley, Colorado, by and through its City Council, on behalf of the Greeley
Police Department, whose address is 1000 10th Street, Greeley, Colorado 80631, hereinafter
referred to as "Greeley"; and the City of Evans,Colorado,by and through its City Council, on behalf
of the Evans Police Department,whose address is 3700 Golden,Evans,Colorado 80620,hereinafter
referred to as "Evans".
WITNESSETH:
WHEREAS, County, District Attorney, United Way, Greeley, and Evans, along with other
agencies and individuals involved in the child abuse and neglect system in Weld County and the
members of the Weld County Child Abuse Coalition, have established a child abuse assessment
center to be known as"A Kid's Place", located on the second floor of the building located at 814
Ninth Street, Greeley, Colorado, and
WHEREAS, A Kid's Place is used by County, District Attorney, Greeley, and Evans in the
course of conducting coordinated investigations into cases of child abuse and neglect, and
WHEREAS, on April 17, 1995, the parties hereto approved an Agreement to define their
respective rights and responsibilities in their use of A Kid's Place, entitled: "Agreement for Use and
Operation of'A Kid's Place' (the"Original Agreement"), and
WHEREAS, the parties hereto now desire to amend the Original Agreement so as to better
define such respective rights and responsibilities,with the amendments being based upon the parties'
collective experiences in using A Kid's Place over the past one year.
NOW, THEREFORE, in consideration of the mutual promises and covenants stated herein,
the parties hereto agree to amend the Original Agreement, as follows:

Amend Section 2.d.6) to read as follows:
2. POLICIES AND PROCEDURES FOR THE USE OF A KID'S PLACE:
d. Access to Investigation Information Gathered at A Kid's Place:
Access to all information which may be gathered through investigations
conducted at A Kid's Place is confidential and is to be used only in the course
of such investigations. Those persons who are authorized to know of such
information are only the following:
6) Any persons designated and/or authorized for access to such
information pursuant to the provisions of the Colorado Children's
Code, including, but not limited to, those persons who have received
authorization by court order and who have signed a Confidentiality
Agreement identical to that attached hereto as Exhibit"A".
Amend Section 5. to read as follows:
5. A KID'S PLACE STAFF:
United Way shall provide one of its employees(the"AKP Coordinator") and various
interns and volunteers to staff A Kid's Place (the "A Kid's Place Staff'). The A
Kid's Place Staff members shall be responsible for the following:
a. Oversight and coordination of A Kid's Place. Such oversight and
coordination shall be limited to those activities necessary to support the
investigations which take place at A Kid's Place, rather than to conduct the
investigations.
b. Scheduling the use of A Kid's Place for all agencies that utilize the facility,
scheduling medical exams (for law enforcement only), record keeping
(demographic information for case tracking to determine effectiveness of
program), and appointment coordination between interviews and medical
exams.
c. Maintenance of equipment and facility.
d. Explaining and providing expertise for the use of video and audio equipment
kept at A Kid's Place.

e. Providing support to A Kid's Place team members, including, but not limited
to:
1) Greeting and providing comfort to persons attending appointments at
A Kid's Place.
2) Attending A Kid's Place team staffings.
3) Setting up mandatory facility and equipment training and
miscellaneous non-mandatory training.
4) Providing the necessary support, coordination, and liaison
for the A Kid's Place Committee.
f. Accomplishing other non-investigatory tasks assigned to the A Kid's Place
Staff by the United Way Supervisor.
The A Kid's Place Staff members shall be considered employees, interns, and/or
volunteers of United Way only and shall not be entitled to any salary or benefits from
the other parties to this Agreement and/or any agencies utilizing A Kid's Place.
United Way agrees to provide all necessary insurance for the A Kid's Place Staff
members, including, but not limited to, unemployment insurance, worker's
compensation insurance, and liability insurance which will cover the activities of the
A Kid's Place Staff members and United Way for any third-party liability claims
alleged to have been caused by the A Kid's Place Staff members while performing
the tasks for A Kid's Place as stated herein.
All other Sections of the Original Agreement shall remain the same.

I, , understand that any information concerning a case of A
Kid's Place I have access to while working as an employee/intern/volunteer is
confidential, and I will not discuss such information with anyone except designated A
Kid's Place Team members, unless other wise required to do so by state or federal laws.
I further understand that all client and case-related names and identifying information are
strictly confidential.
All personnel and financial matters relating to A Kid's Place activities are likewise
confidential and require the same non-disclosure.
