RE: APPROVE CHANGE ORDER LETTER NO. 1 FOR CANCER PREVENTION AND
CONTROL PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter No. 1 for Cancer
Prevention and Control Program between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, Prevention Division,
commencing August 1, 1995, and ending July 15, 1996, with further terms and conditions being
as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Change Order Letter No. 1 for Cancer Prevention and Control Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, Prevention Division, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 14th day of February, A.D., 1996, nunc pro tunc July 15, 1995.

Persuant to paragraph 10, Contract PPG950409, Routing Number 961375, (copy attached and by this reference made a part
hereof) between the State of Colorado, Department of Public Health and Environment, Prevention Division, Cancer Prevel* e 7. /
and Control Prog and Weld County Department of Health for the maximum amount of$48;4B8-(Eigltteen-Thousandio

at-Dollars) is hereby decreased by $8,062(Eight Thousand Sixty Two Dollars) to a new total of$10,366
(Ten Thousand Three Hundred Sixty Six Dollars) per paragraph 9. A copy of the new budget is attached as revised Exhibit M.
The terms and conditions of the original contract shall remain the same.
This letter of amendment to the contract is intended to be effective January 15, 1996, but in no event shall this amendment be
deemed valid until it shall have been approved by the State Controller or such assistant as he may desienate.
Please sign all four(4) copies of this letter and return to my attention by January 30, 1996 at:

Subject: Change Order Letter No. 1 to Cancer Prevention Contract
Enclosed for Board review is change order letter No. 1 between the Weld County Health
Department and the Colorado Department of Public Health and Environment for the Cancer
Prevention Contract for low income women over the age of 40.
This change order letter reduces the amount of the current contract from $18, 471 to $10, 366 for
the period August 1, 1995 through July 15, 1996. This action is being taken because of recent
furloughs of federal CDC employees which resulted in their inability to process expanded
budgets. The Colorado Department of Public Health and Environment has assured us this
reduction in funding is temporary and we will be returned to our full funding within a short
period of time.
I recommend acceptance of this change order.
