The following comments are recommended to be a part of any approval: 1. The I-25 Frontage Road access falls under the State's jurisdiction. Please contact Tess Jones, Colorado Department of Transportation at the Greeley Office, to verify the new or existing access and drainage at this location. 2. Four inches of aggregate base course needs to be placed on the north access road. This road needs to be graded and drained to accommodate the facility. 3. Batteries in sufficient quantity require reporting under the Superfund Amendment and Reauthorization Act (SARA) Title III of 1986. If their are no chemical on site then I would have no problems with this USR. If there will be chemicals over the threshold planning quantity then I would request that they be required to prepare a plan and annual reports per Section 302, 311 and 312. The Department supports the location of this access provided there is adequate sight distance in both directions. The culvert size as shown on the plans is unacceptable. Culverts within the state highway right-of-way must be 18" in diameter. Also, the access must be constructed at least 16' wide. The applicant should contact me to obtain an access permit. UTILITIES If new utility lines or connections are necessary within the state highway right-of-way, a utility permit will be required. Prior to such utility work, Mr. Henry Rangel of the Department should be contacted at (970) 350-2111. Our comments are based upon the submittal as received August 12, 1996. Greeley, CO 80631 RE: USR-1126 KTS/WIRELESS PCS III Dear Mr. Schuett : I have reviewed the submitted application materials for the above referenced project . This project will be located within the boundaries of the Fire District and will receive service from the Fire District . The Fire District has no objections to this project providing the adopted codes and standards of the Fire District can be met . I have the following comments : 1 . The Fire District' s Standard for fire apparatus access roads is an all-weather surface which shall be a minimum of twenty feet wide, shall have an unobstructed clearance of at least thirteen feet, six inches (13 ' 6" ) , and shall be capable of supporting the imposed load of fire apparatus weighing at least sixty thousand (60 , 000) pounds . It appears as though the proposed road width meets the above requirements . If fire apparatus access roads are not looped in such a way as to allow driving completely around the building or complex of buildings, then the dead-end fire apparatus access roads shall meet the requirements of the following Table 3 . 04 : Refer to copy enclosed. Due to the nature of this project, the Fire District will accept the options listed for a roadway length between 151-500 feet . 2 . Install a padlock from the Knox Company for fire department access to the interior of the fenced area. Key access is only available to Fire District Command Personnel . Contact the Fire Marshal for an application form which will need to be completed and returned with a check to the Knox Company. The Fire Marshal will then provide an authorized signature on the application and send the order in. The padlock will then be shipped directly to the applicant . Applicant is to notify the Fire Marshal when padlock is placed into service . Buildings more than one ( 1 ) story in height shall be designed in such a manner that ground ladder operations with access to the interior can occur on the same side of the building as the fire apparatus access route. 3 . 03-2 . Buildings or portions of buildings exceeding thirty- five ( 35 ) feet in height from the lowest point of fire department access shall be provided with a fire access route capable of accommodating Fire Department aerial apparatus . Required access roadways shall be a minimum twenty-six ( 26 ) feet in width in the immediate vicinity of any building or portion of building more than thirty-five ( 35 ) feet in height. At least one of the required access roadways meeting this condition shall be located within a minimum of fifteen ( 15 ) and a maximum of twenty-five ( 25 ) feet from the building, and shall be positioned parallel to one entire side of the building. Note: The measurement for this requirement shall be taken from the exterior wall of the building to the nearest edge of the fire apparatus access road. 3 . 04 Dead-end Fire Apparatus Access Routes 3 . 04-1 . All liquid and solid wastes shall be stored and removed for final disposal in a manner that protects against surface and groundwater contamination. 2. No permanent disposal of wastes shall be permitted at this site. 3. Waste materials shall be handled, stored, and disposed in a manner that controls fugitive dust, blowing debris, and other potential nuisance conditions. 4. Fugitive dust shall be controlled on this site. 5.
