RESOLUTION
RE: APPROVE CHANGE ORDER LETTER #1 FOR IMMUNIZATION PROGRAM
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#1 to the
Immunization Program Contract between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment, commencing upon full execution of said change order, and ending December
31, 1999, with further terms and conditions being as stated in said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Change Order Letter#1 to the Immunization Program Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 3rd day of November, A.D., 1999.

SUBJECT: Change Order Letter#1 to the Immunization Program Contract
DATE: October 29, 1999
Enclosed for Board review and approval is Change Order Letter#1 to the Immunization Program
Contract between the Colorado Department of Public Health and Environment (CDPHE) and
Weld County Department of Public Health and Environment (WCDPHE).
The Immunization Program at CDPHE recently received additional funding from the Centers for
Disease Control and Prevention and is passing some of this funding on to local health
departments throughout the State. Weld County will receive an additional $2,234 which can be
used for any activities or supplies that will enhance the County's immunizations efforts for
children.
This increase in funding will bring the total reimbursement of the contract to $29,234 for the
time period January 1, 1999 through December 31, 1999.
I recommend your approval of this change order letter.

State Fiscal Year 2000-2001 Change Order Letter No. 1 Contract Routing Number 00 FHA 00117
Pursuant to paragraph 3 of Section "C"of the contract with contract routing number 99 FHA 00077 and
contract encumbrance number PO FHA EPI9900077 hereinafter referred to in total as"the Original Contract"
(a copy of which is attached hereto and by this reference incorporated herein and made a part hereof) between
the State of Colorado, Department of Public Health and Environment and Weld County Health Department,
for the term from January 1, 1999,through December 31, 1999,the parties agree that the maximum amount
payable by the State for the eligible services in Paragraph 1 of Section "C" of the Original Contract is
increased by TWO THOUSAND TWO HUNDRED THIRTY FOUR DOLLARS ($2,234.00) for a new total
of TWENTY NINE THOUSAND TWO HUNDRED TWENTY FOUR DOLLARS ($29,234.00). The first
sentence in paragraph 1 OF Section "C" of the Original Contract is hereby modified accordingly. All other
terms of conditions of the Original Contract are hereby reaffirmed. This amendment to the Original Contract is
intended to be effective as of November 15, 1999,but in no event shall it be deemed valid until it shall have
been approved by the State Controller or such assistant as he may designate.

original of this letter will be returned to you when fully approved.
