HomeMy WebLinkAbout981455.tiff RESOLUTION RE: APPROVE TWO NOTIFICATION OF FINANCIAL ASSISTANCE AWARDS FOR FAMILIES, YOUTH AND CHILDREN COMMISSION (CORE) FUNDS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with two Notification of Financial Assistance Awards for Families, Youth and Children Commission (Core) Funds between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Lutheran Family Services, Intensive Family Therapy, commencing June 1, 1998, and ending May 31, 1999, with further terms and conditions being as stated in said awards, and WHEREAS, after review, the Board deems it advisable to approve said awards, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex-officio Board of Social Services, that the two Notification of Financial Assistance Awards for Families, Youth and Children Commission (Core) Funds between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and Lutheran Family Services, Intensive Family Therapy, be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said awards. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 17th day of August, A.D., 1998, nunc pro tunc June 1, 1998. Harbert, Chair Board of County Commissioners FROM: Judy Griego, Director, Social Services.' of DATE: August 10, 1998 SUBJECT: Revision of Notification of Financial As [stance Awar (NOFAA) 98-CORE-0001 (RFP-FYC-98010) and 98-CORE-0002(RFP-FYC-98008) Lutheran Family Services Enclosed for Board approval are revised FY98-99 Notifications of Financial Assistance Awards (NOFAA) for Families, Youth and Children Commission(FYC) Core Services Funds, which are for the period of June 1, 1998, through May 31, 1999. The Families, Youth and Children Commission(FYC) reviewed the revised rate adjustments through the Effectiveness and Outcomes Committee and is recommending approval of the rate adjustments. The rate adjustments were requested by Lutheran Family Services to include into their direct cost hourly unit rate. Lutheran Family Services Intensive Family Therapy-LIFE Program The LIFE program is designed to promote the safety and well being of children and their family members. Services will build upon primary family connections and to help parents improve their parenting ability, identify parental strengths and support their efforts to provide care for their children. The program goal is to improve both individual and family functioning. Crisis intervention services will be provided throughout the duration of the families' involvement with our agency. Maximum number of families served per year is 32 with children 0-18 years, a maximum of eight families at any given time. Hourly rate per unit of service is $100.00. Services will build upon primary family connections and help parents improve their parenting ability, identify parental strengths and support their efforts to provide care for their children. The program goal is to improve both individual and family functioning. Crisis intervention services will be provided throughout the duration of the families' involvement with our agency. Maximum number of families served per year is 32 with children 0-18 years, a maximum of eight families at any given time. Hourly rate per unit of service is $96.00.
