RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - FRANK M. ANDREWS, PH.D.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and Frank M. Andrews, Ph.D., commencing June 1, 1998, and ending August 31,
1998, with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Amendment to Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and Frank M. Andrews, Ph.D., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of August, A.D., 1998, nunc pro tunc June 1, 1998.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT
OF SOCIAL SERVICES AND FRANK M. ANDREWS, PH. D.

This Amendment, made and entered into the th day of June 1998, by and between the Board of
County Commissioners, on behalf of the Weld County Department of Social Services, hereinafter referred to as
"Social Services,"and Frank M. Andrews, Ph. D., hereinafter referred to as the"Contractor."
WITNESSETH
WHEREAS,the parties desire to modd'y their current contract for child protection services(PY98-CPS-2)
in order to expand the scope of services;and
WHEREAS,the authority exists to disburse funds in the Family Preservation Program and Child Welfare
Administration, funds have been budgeted, appropriated, and otherwise made available; and
WHEREAS, required approval, clearance, and coordination have been accomplished from and with
appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:'
1. This Amendment shall be effective June 1, 1998.
2. The term of this Amendment shall be extended to August 31, 1998.
3. ExhIit B Payment Schedule,shall be revised to include two sources of funds, mental health core
service funds under the Family Preservation Program or Child Welfare Administration.
4. Hourly service rate of $90.00 will remain with a total expenditure for the period of the
Amendment (June 1, 1998,through August 31, 1998) not to exceed-46,000.00. ?PM Cs "/
Except as modified herein, the terms and conditions of the original agreement (PY98-CPS-2) and
modifications to the original agreement (PY98-CPS-2), together with all attachments of the original agreement
remain in full force and effect.

IN WITNESS WHEREOF,the parties hereto have duly executed the Agreement as of the day, month,
and year first above written.

Enclosed for Board approval is an Amendment to:
• PY98-CPS-2 Child Protection Agreement for Services Between the Weld
County Department of Social Services and Frank M. Andrews,Ph. D.,
effective from June 1, 1998, to August 31, 1998.
• This Amendment provides consultation to foster parents.
The purpose of the Amendment is to:
a. Extend the duration of the Agreement to August 31, 1998. This extension
has been planned and approved by North Range Behavioral Health.
b. Increase the maximum allowable reimbursement total from $11,700
to $19,700.
The other terms of the Agreement remain the same.
