PURSUANT TO C.R.S., §24-10-109
1. Name and Address of Claimant: Paula Ikenouye, 4411 W. 1st
Street, Greeley, Colorado 80634.
Name and Address of Claimant's Attorney: R. Russell Anson,
820 13th Street, Greeley, Colorado 80631.
2. Concise Statement of Factual Basis of Claim: The following
facts are presented to the best of Claimant's knowledge:
On August 7, 1996, William Ikenouye, an electrician, while
performing services for a customer at 31269 3rd Street, Gill,
Colorado 80624, died by electrocution due to faulty wiring on
those premises. The death of William Ikenouye was the result
of negligence of Weld County Building Inspection employees, in
their failure to require correction of faulty wiring on the
premises at a time when they knew or should have known such
faulty wiring existed, due to prior visits to the premises.
3. Name and Address of any Public Employee involved: This is
unknown at this time, although it would be an employee, or
former employee of the Weld County Building Inspection Office.
4. A Concise Statement of the Nature and the Extent of the Injury
Claimed to Have Been Suffered: Electrocution resulting in
death. Grief, loss of companionship, pain and suffering, and
emotional distress to the Claimant and her children.
5. Amount of Monetary Damage Being Requested:
Claimant is seeking damages for wrongful death. A minimum of
$1,000,000.00 for economic losses, and a maximum of non-
economic loss or injury as defined under C.R.S. §13-21-203,
including damages for grief, loss of companionship, pain and
suffering, and emotional stress to the surviving parties up to
$250,000.00
