RE: APPROVE CONTRACT RENEWAL LETTER FOR PRENATAL PROGRAM AND
AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract Renewal Letter for the Prenatal
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, commencing October 1, 1995, and ending
September 30, 1996, with further terms and conditions being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Contract Renewal Letter for the Prenatal Program between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Health Department, and the Colorado Department of Public Health
and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chairman be, and hereby is, authorized
to sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 25th day of October, A.D., 1995, nunc pro tunc October 1, 1995.

CONTRACT RENEWAL LETTER

The Colorado Department of Public Health and Environment hereby notifies Weld
County Health Department that pursuant to paragraph 12 of contract number
PRN950536, the state will exercise its option to renew this contract for an
additional one year period beginning October 1, 1995 through September 30, 1996.
Paragraph 9 of the contract is deleted and replaced with the following
(paragraphs 9a and 9b remain unchanged):
The State will pay Forty-two thousand five hundred ($42,500) Dollars to
administer the program, purchase medical supplies, etc and provide education and
counseling to 100 patients; Thirty thousand ($30,000) Dollars for diagnostic
tests; and Thirty-three thousand seven hundred fifty ($33,750) Dollars for
PRENATAL PLUS Enhanced services to 75 patients. In consideration of said
services by the Contractor and conditioned upon affirmation by the Women's Health
Section Prenatal Program that services were rendered in accordance with this
contract, the State will cause to be paid to the Contractor a sum not to exceed
One hundred six thousand two hundred fifty ($106,250) Dollars as follows:
This contract renewal letter is intended to be effective October 1, 1995, but in
no event shall it be deemed valid until it is approved by the State Controller
or his designee.
Please sign and return all copies of this notification. A fully executed copy
will be returned to you.

Contract Renewal Letter for Prenatal Program
Enclosed for Board approval is a contract renewal letter between Weld County Health
Department and the Colorado Department of Public Health and Environment for our prenatal
program.
Under the provisions of this agreement, the Health Department will provide prenatal and
postpartum care, including education and counseling, for 100 non -Medicaid clients and case
management services to 75 clients during the period October 1, 1995 through September 30,
1996. For these services, the Health Department will be reimbursed $106,250. This is a $32,150
decrease compared to last year's contract. The reasoning behind the cut in funding from CDPHE
is Medicaid will pay the Health Department for case management services beginning in the spring
of 1996.
I recommend approval of this renewal letter.
