RE: APPROVE IMMUNIZATION ACTION PLAN CONTRACT AMENDMENT #1 BETWEEN
THE WELD COUNTY HEALTH DEPARTMENT AND THE COLORADO
DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT AND AUTHORIZE
CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with the Immunization Action Plan Contract
Amendment #1 between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Health Department, and
the Colorado Department of Public Health and Environment, commencing July 1, 1997, and
ending December 31, 1997, with further terms and conditions being as stated in said
amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Immunization Action Plan Contract Amendment #1 between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the Colorado Department
of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 18th day of June, A.D., 1997.

AMENDMENT #1
THIS AMENDMENT, made this 1st day of May, 1997 by and between the State of Colorado
for the use and benefit of the Department of Public Health and Environment, 4300 Cherry
Creek Drive South, Denver, Colorado 80222-1530 hereinafter referred to as the State, and
Weld County Health Department, 1517 16th Avenue Court, Greeley, Colorado, 80631
hereinafter referred to as the Contractor.
WHEREAS, authority exists in the Law and Funds have been budgeted, appropriated
and otherwise made available and a sufficient unencumbered balance thereof remains available
for payment in Fund Number 100, APPR code 351, Contract Encumbrance Number FHA
EPI9605174; and
WHEREAS, the State and Distance Control and Environmental Epidemiology Division
of the Colorado Department of Public Health and Environment (CDPHE) has received
additional funding from the U.S. Department of Health and Human Services, Centers for
Disease Control and Prevention (CDC) for an Immunization Action Plan to increase and
enhance the availability of age -appropriate immunizations for children; and
WHEREAS, the parties entered into a contract dated September 25, 1995, Contract
Encumbrance Number EPI9605174, Contract Routing Number 96-05174, as amended by
Change Order Letter No. 1, routing number 97-05233, and Renewal Letter No. 1, routing
number 97-06363, herinafter referred to as "Original Contract" whereby contractor was to
provide certain services to the State; and
WHEREAS, it is the intension of the State to increase the Original Contract,
EPI9605174 as amended, by Sixty -Eight Thousand Dollars ($68,000.00) to a new total of One
Hundred Sixty -Eight Thousand Dollars ($168,000.00) for the provision of additional services
for the time period between July 1, 1997 and December 31, 1997.
NOW THEREFORE, it is hereby agreed that for and in consideration of their mutual
promises to each other, hereinafter stated, the parties hereto agree as follows:
1. Consideration for this amendment to the original contract, Contract Encumbrance
number EPI9605174, Contract Routing number 96-05174, dated September 25, 1995, as
amended, consists of the payments which shall be made pursuant to this agreement and the
promises and agreements herein set forth.

2. It is expressly agreed by the parties that this contract is supplemental to the original
contract EPI9605174, routing number 96-05174 dated September 25, 1995, as amended, which
is, by this reference, incorporated herein and made a part hereof, and is attached hereto as
Attachment A. All terms, conditions, and provisions of the Original Contract, unless specifically
modified herein, are to apply to this Amendment as though they were expressly rewritten,
incorporated, and included herein.
3. It is agreed that the original contract EPI9605174, as amended, between the State of
Colorado and the Weld County Health Department, shall be, and is hereby modified, altered, and
changed in the following respects only:
A. By adding to the Original Contract, EPI9605174, as amended , the following
paragraph and budget:
14. The following budget shall govern the expenditure of funds for the period
between January 1, 1997 and December 31, 1997 by the Contractor as
well as the subsequent reimbursement by the State. Transfer of funds from
one line item to another must have written approval by the State:
Personnel for two (2) full time nurses
(includes fringe and indirect)
Personnel for Tracking Secretary
(includes fringe and indirect)
Travel to Immunization Agencies
Overnight trips
Total (January 1, 1997 - December 31, 1997)

B. By adding to the original contract, EPI9605174, as amended, the following paragraphs:
15. One (I) Weld County Health Department Public Health Nurse Coordinator
will continue to provide technical assistance to public health agencies
identified as still lagging behind at less than 80% vaccination coverage
levels, The IAP nurse will also be part of the Immunization Technical
Assistance Team (ITAT). Interventions will focus on working with clinic
staff to identify immunization barriers; and then, designing and
implementing quality improvement activities (AFIX, reminder/recall and
computerized tracking), including regular periodic technical assistance
visits to ensure progress. Contractor will ensure that adequate travel funds
and long distance phone call services are available to fulfill this objective.
16. Under the direction of the Weld County Health Department Nurse
Coordinator described above, and with the cooperation of the Colorado
Department of Public Health and Environment Immunization Program, two
(2) full time nurses will work with clinic staff to identify immunization
barriers, then design and implement quality improvement activities (AFIX,
reminder recall and computerized tracking), including regular periodic
technical assistance visits to ensure progress. Furthermore, these nurses
will assist in developing relationships with private sector providers. One of
these nurses will work in the Eastern plains of Colorado (in the vicinity of
the Weld County Health Department), and one nurse will be located in the
western slope of Colorado (to be decided between: Mesa County Health
Department, Delta County Health Department, and San Juan Basin Health
Department). The term of this project is July 1, 1997 through December
31, 1997.
4. The effective date of this amendment is May 30, 1997, but in no event shall be deemed
valid until approved by the State Controller or such assistant as he may designate.
5. Except for the "General Provisions" and "Special Provisions", in the event of any conflict,
inconsistency, variance, or contradiction between the provisions of this amendment and any of the
provisions of the Original Contract EPI9605174, the provisions of this amendment shall in all
respects supersede, govern and control. The "Special Provisions" shall always be controlling over
other provisions in the contract or amendments. The representations in the Special Provisions
concerning the absence of bribery or corrupt influences and personal interest of State employees
are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT
FISCAL YEAR ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING
APPROPRIATED, BUDGETED, AND OTHERWISE MADE AVAILABLE.
7. THIS AMENDMENT SHALL NOT BE DEEMED VALID UNTIL IT SHALL HAVE
BEEN APPROVED BY THE CONTROLLER OF THE STATE OF COLORADO OR SUCH
ASSISTANT AS HE MAY DESIGNATE.

Enclosed for Board approval is Amendment #1 to the Immunization Action Plan Contract
between the Weld County Health Department and the Colorado Department of Public
Health and Environment.
This amendment provides funding for the Health Department to employ two additional
full-time temporary nurses who will function as advisors to Colorado eastern plains and
western slope public health agencies for improving the immunization status of children
living in those areas. These two temporary nurses will work under the direction of
Rosemary Spence, the Weld County Health Department nurse who is the coordinator of
the Immunization Action Plan.
For these services, WCHD will be reimbursed an amount not to exceed $68,000 for the
salary, fringe benefits, travel and other operating expenses for two temporary nurses for
the time period July 1, 1997 through December 31, 1997.
This program continues to focus on a critical public health population, and Health
Department staff recommends your approval of this amendment.
