RE: APPROVE CONTRACT RENEWAL LETTER NO. 1 BETWEEN HEALTH
DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC HEALTH AND
ENVIRONMENT (DISEASE CONTROL AND ENVIRONMENTAL EPIDEMIOLOGY
DIVISION) AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter No. 1 between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the Colorado Department
of Public Health and Environment (Disease Control and Environmental Epidemiology Division),
commencing January 1, 1997, and ending December 31, 1997, with further terms and
conditions being as stated in said contract renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said contract renewal
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Contract Renewal Letter No. 1 between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Weld County Health Department, and the Colorado Department of Public Health and
Environment (Disease Control and Environmental Epidemiology Division) be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 12th day of March, A.D., 1997, nunc pro tunc January 1, 1997.

In accordance with Paragraph 13 of contract routing number 96051710f contract number EPI960 5174
contract routing number 97-05233 of Change Order Letter No. 1 (copies attached and by this reference made
a part hereof) between the State of Colorado Department of Public Health and Enviornment (Disease Control
and Environmental Epidemiology Division) and Weld County Health Department covering the period of
January 1, 1997 through December 31, 1997 the undersigned agree that the maximum amount payable by the
State for eligible services is One Hundred Thousand Dollars ($100 000),

original contract shall remain the same.
This amendment to the contract is intended to be effective as of 01/01/97, but in no event shall it he deemed
valid until it shall have been approved by the State Controller or such assistant as he may designate

Immunization Contract Renewal Letter #1

Enclosed for Board review and approval is Contract Renewal Letter #1 to the
Immunization Action Plan Contract between the Weld County Health Department and the
Colorado Department of Public Health and Environment.
Under the provisions of the contract and this renewal letter, WCHD will receive up to
$100,00 for the period January 1, 1997 through December 31, 1997 for services provided
to increase the immunization levels for children aged birth to two years of age in Weld
County. Also, a WCHD nurse, Rosemary Spence, will provide technical assistance to
other public health agencies throughout the state which have been identified as having
less than 80% vaccination coverage levels. This funding is an increase of $55,619 over
the previous years contract. The increase will pay for travel expenses for Ms. Spence as
well as salary and fringe benefits for a nurse to assume her duties at Weld County Health
Department for one year.
This program continues to focus on a critical public health population, and I recommend
your approval of this renewal letter.
