RE: APPROVE CHANGE ORDER LETTER NO. 3 FOR CHLAMYDIA TESTING CONTRACT
BETWEEN HEALTH DEPARTMENT AND COLORADO DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Change Order Letter No. 3 for Chlamydia
Testing Contract between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, commencing January 1, 1996, and ending
December 31, 1996, with further terms and conditions being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of which
is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Change Order Letter No. 3 for Chlamydia Testing Contract between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Weld County Health Department, and the Colorado Department of Public
Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 6th day of March, A.D., 1996, nunc pro tunc January 1, 1996.

Change Order Letter No. 3
In accordance with Paragraph 5 of routing number 94-0876 of Contract No. EPI940876, routing
number 95-1523 of Letter No. 1, and routing number 9605118 of Letter No. 2 (copies attached
and by this reference made a part hereof) between the State of Colorado Department of Public
Health and Environment(Disease Control and Environmental Epidemiology Division)and Weld
County Health Department covering the period of January 1, 1996 through December 31,
1996, the undersigned agree that the maximum amount payable by the State for eligible services
in Paragraph 3 is being increased by $2,940 to a new total of$3,675 to be spend accordingly
to the revised budget outlined below. Funding is contingent upon continued availability of
federal funds for payment pursuant to the terms of this agreement. The terms and conditions
of the original contract shall remain the same.
Family Planning Clinic chlamydia tests
(240 @ $7.50 each) $ 1,800
STD Clinic chlamydia tests
(women and male contacts)
(250 @ $7.50 each) $ 1,875
REVISED TOTAL $ 3,675
This amendment to the contract is intended to be effective as of 03/15/96, but in no event shall
it be deemed valid until it shall have been approved by the State Controller or such assistant as
he may designate.

Subject: Change Order Letter No. 3 for Chlamydia Testing Contract
Enclosed for Board review and approval is Change Order Letter No. 3 for the Chlamydia Testing
contract between the Weld County Health Department and the Colorado Department of Public
Health and Environment.
This contract pays for chlamydia tests done in our sexually transmitted disease and family
planning clinics. In January of this year, you approved Change Order Letter No. 2 which was
written for only $735 or 20% of the predicted funding for the year. That change order was issued
because of the federal budget delay. This change order letter is restoring the contract to the full
amount of$3,675 which is a$562 increase over last year's funding.
I recommend your approval of this change order letter.
