RE: APPROVE AMENDMENT TO WORKING AGREEMENT FOR ALIVE/E PROGRAM
WITH COLORADO DEPARTMENT OF HUMAN SERVICES AND AUTHORIZE CHAIR
TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Working Agreement
for ALIVE/E Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and the Colorado Department of Human Services, commencing July 1, 1996, and
ending June 30, 1997, with further terms and conditions being as stated in said amendment,
and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Amendment to Working
Agreement for ALIVE/E Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Social Services, and the Colorado Department of Human Services be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 25th day of November, A.D., 1996, nunc pro tunc July 1, 1996.

AMENDMENT TO
WORKING AGREEMENT
THIS AMENDMENT made and entered into this 25th Day of November 1996, by and between
the Colorado Department of Human Services, hereinafter referred to as the "State," and the Weld
County Department of Social Services, hereinafter referred to as the "County":
WITNESSETH
WHEREAS, the parties desire to modify their current working agreement for ALIVE/E services in
order to increase its funding; and
WHEREAS, the authority exists in the Law and funds have been budgeted, appropriate, and
otherwise made available; and
WHEREAS, required approval, clearance, and coordination have been accomplished from and
with the State and the County.
NOW THEREFORE, in consideration of the premises, the parties hereto covenant and agree as
follows:
1. Section V, Payment, Item B. The sentence shall be changed to include the increase in
funding and shall read, "The position is classified as a Caseworker III. Associated costs and
fringe benefits are built in as a block grant to a maximum of$41,000 for this twelve month
period".
2. Exhibit Budget, Funds for ALIVE/E Program 1000/0 Federally Funded Independent Living
Position, Associated Costs/2FTE. The computation for this section has been reduced as
follows:
Operating (phone, supplies, etc.) $1,695
Travel (actual @ .20/mile, estimated up to) $2,500
Capital Outlay $ 100
$4,295
3. Exhibit Budget, Funds for ALIVE/E Program 1000/0 Federally Funded Independent Living
Position, First Paragraph. Salary will be reimbursed at an actual rate. Total reimbursement
cannot exceed $41,000/FTE. Consideration must be given for future salary progression in
order to remain under the cap of$41,000/FTE. For the position funding as 0.5 FTE, the
amount is $20,500.
Except as modified herein, the terms and conditions of the original working agreement together
with all exhibits of the original working agreement, remain in full force and effect.

COLORADO Between the Weld County Department of Social
Services and the Colorado Department of Human
Services
Enclosed for Board approval is an amendment to the ALIVE/E Program Working Agreement
• between the Weld County Department of Social Services and the Colorado Department of Human
Services. The Agreement is for the period of July 1, 1996 through June 30, 1997.
The purpose of Amendment is to incorporate final State allocations which will increase the
reimbursement total from $40,000/FTE to $41,000/FTE.
