RE: APPROVE CHANGE ORDER LETTER NO. 4 FOR CHLAMYDIA TESTING
CONTRACT BETWEEN THE WELD COUNTY HEALTH DEPARTMENT AND THE
COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter No. 4 for
Chlamydia Testing Contract between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department and the Colorado Department of Public Health and Environment, covering the
period of January 1, 1996, through December 31, 1996, with said amendment becoming
effective as of October 1, 1996, with further terms and conditions being as stated in said
change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Change Order Letter No. 4 for Chlamydia Testing Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department and the
Colorado Department of Public Health and Environment, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.

Change Order Letter No. 4
In accordance with Paragraph 5 of routing number 94-0876 of Contract No. EPI940876, routing
number 95-1523 of Letter No. 1, routing number 96-1606 of Letter No. 2 and routing number
96-05098 of Letter No. 3 (copies attached and by this reference made a part hereof) between the
State of Colorado Department of Public Health and Environment (Disease Control and
Environmental Epidemiology Division) and Weld County Health Department covering the
period of January 1, 1996 through December 31, 1996, the undersigned agree that the maximum
amount payable by the State for eligible services in Paragraph 3 is being increased by $2,500
to a new total of$6,175 to be spend accordingly to the revised budget outlined below. Funding
is contingent upon continued availability of federal funds for payment pursuant to the terms of
this agreement. The terms and conditions of the original contract shall remain the same.

This amendment to the contract is intended to be effective as of 10/01/96, but in no event shall
it be deemed valid until it shall have been approved by the State Controller or such assistant as
he may designate.

Subject: Change Order Letter No. 4 for Chlamydia Testing Contract
Enclosed for Board review and approval is Change Order Letter No. 4 for the Chlamydia Testing
contract between the Weld County Health Department and the Colorado Department of Public
Health and Environment.
This contract reimburses the Health Department for chlamydia tests done in our sexually
transmitted disease and family planning clinics. This change order letter increases the maximum
amount payable for the time period January 1, 1996 through December 31, 1996 by $2,500 to a
new total of$6,175 which is an increase of$3,062 over last year's funding.
I recommend your approval of this change order letter.
