RE: APPROVE PURCHASE OF SERVICES AGREEMENT WITH CENTENNIAL
DEVELOPMENT SERVICES, INC., AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, and
Centennial Development Services, Inc., with terms and conditions being as stated in said
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Department of Human Services, and Centennial Development Services, Inc., be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 22nd day of December, A.D., 1997.

PURCHASE OF SERVICES AGREEMENT
THIS AGREEMENT, made and entered into this 1 St. day of November 1997 by and between the Weld County
Division of Human Services, hereinafter referred to as "HS" and Centennial Developmental Services, Inc., hereinafter
referred to as "CDSI."
WITNESSETH:
WHEREAS, HS administers a door—to—door transportation system for the low income, elderly, and disabled, and
WI IEREAS, CDSI is a non—profit community incorporated board which provides services for the developmentally
disabled, and
WHEREAS, CDSI is desirous of entering into an agreement with the HS Transportation Program for short term
transportation services while the agency is preparing to initiate its own transportation system.
Now THEREFORE, it is mutually agreed as follows:
1. HS agrees to provide interim transportation services to CDSI until such time the CDSI transportation service is
operational.
2. HS agrees to provide service and maintain vehicles in compliance with local, state, and federal safety and transit
guidelines.
3. CDSI agrees to reimburse HS at $1.60 per mile to cover actual overhead, gas and oil, repair and maintenance,
salaries, and vehicle replacement.
4. HS agrees to bill CDSI within 15 days of the closing date of each monthly financial period.
5. CDSI agrees to reimburse HS monthly and with 30 days of the receipt of each billing.
6. This Agreement may be amended with the written consent of both parties.
IN WITNESS WHEREOF, the parties hereto have executed this Agreement as of the date first written above.

Purchase of Service Agreement with Centennial Developmental
Services, Inc.
Enclosed for Board approval is an Agreement with Centennial Developmental Services, Inc. (CDSI).
This Agreement replaced the Agreement that was terminated by CDSI effective October 31, 1997.
The Weld County Transportation Program will be providing limited transportation services to CDSI
under this Agreement as CDSI will be providing transportation for their clients with their own vans.
The per mile rate to CDSI has increased from $1.20 per mile to $1.60 per mile under the new
Agreement.
