RE: APPROVE CONTRACT AMENDMENT #1 FOR HEALTH EDUCATION STANDARDS
GRANT WITH THE COLORADO DEPARTMENT OF PUBLIC HEALTH AND
ENVIRONMENT AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Amendment #1 for the Health
Education Standards Grant between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, commencing
March 1, 1997, and ending February 28, 1998, with further terms and conditions being as
stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Contract Amendment #1 for the Health Education Standards Grant
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 28th day of July, A.D., 1997, nunc pro tunc March 1, 1997.

CONTRACT AMENDMENT #1
THIS AMENDMENT, made this 1st day of August, 1997, by and between the State of Colorado for the
use and benefit of the Department of Public Health and Environment, 4300 Cherry Creek Drive South,
Denver, Colorado 80222-1530, hereinafter referred to as the State, and Weld County Health Department,
1517 16th Avenue Court, Greeley, Colorado 80631, hereinafter referred to as the contractor.
WHEREAS, authority exists in the Law and Funds have been budgeted, appropriated, and otherwise
made available and a sufficient unencumbered balance thereof remains available for payment in Fund
Number /00 , Appropriation Code F , Contract Encumbrance Number
ADM9705838; and
WHEREAS, required approval, clearance, and coordination has been accomplished from and with
appropriate agencies; and
WHEREAS, the parties entered into a contract dated March 1, 1997, Contract Encumbrance Number
ADM9705838, Contract Routing Number 9705838, hereinafter referred to as "Original Contract",
whereby contractor was to provide certain services to the State; and
WHEREAS, the State desires to increase the contract amount and pay contractor for additional services
performed during the contract period and the contractor has agreed to the performance of these services.
NOW THEREFORE, it is hereby agreed that:
1. Consideration for this Amendment to the Original Contract, Contract Encumbrance Number
ADM9705838, Contract Routing Number 9705838, dated March 1, 1997, consists of payments made
herein and pursuant to the Original Contract and the promises and agreements herein set forth.
2. It is expressly agreed by the parties that this Amendment is supplemental to the Original
Contract, Contract Encumbrance Number ADM9705838, dated March 1, 1997, which is, by this
reference, incorporated herein and made a part hereof, and is attached hereto as Exhibit ONE. All terms,
conditions, and provisions of the Original Contract, unless specifically modified herein, are to apply to
this Amendment as though they were expressly rewritten, incorporated, and included herein.
3. It agreed the Original Contract is and shall be modified, altered, and changed in the following
respects only:
a. The contract amount, stated on page 1, in paragraph 5, is hereby modified and increased
by $2,204.34 to a new contract total amount of $24,430.34 to provide additional staff
training to implement health education standards.

b. The terms of the Original Contract (including its Attachment A - Scope of Work) are
modified accordingly.
4. The effective date of this Amendment is August 1, 1997.
5. Except for the "General Provisions" and "Special Provisions", in the event of any conflict,
inconsistency, variance, or contradiction between the provisions of this Amendment or any of its
attachments or exhibits, and any of the provisions of the Original Contract, the provisions of this
Amendment shall in all respects supersede, govern, and control. The "Special Provisions" shall always
be controlling over other provisions in the contract or amendments. The factual representations in the
Special Provisions concerning the absence of bribery or corrupt influences and personal interest of State
employees are presently reaffirmed.
6. FINANCIAL OBLIGATIONS OF THE STATE PAYABLE AFTER THE CURRENT FISCAL
YEAR ARE CONTINGENT UPON FUNDS FOR THAT PURPOSE BEING APPROPRIATED,
BUDGETED, AND OTHERWISE MADE AVAILABLE.
7. THIS AMENDMENT SHALL NOT BE DEEMED VALID UNTIL IT SHALL HAVE BEEN
APPROVED BY THE CONTROLLER OF THE STATE OF COLORADO OR SUCH ASSISTANT AS
HE MAY DESIGNATE.

Amendment #1 to the Health Education Standards Contract
Enclosed for Board approval is Amendment # 1 to the Health Education Standards
Contract between Weld County Health Department and the Colorado Department of
Public Health and Environment for the use of Preventive Health and Health Services
Block Grant funds to coordinate and support efforts to enhance health education at state
and local levels.
This amendment will provide additional funding of up to $2,204.34 which is carry over
from the 1996-1997 contract. The funds will be used for training and travel for the full-
time staff member who is coordinating the project. This person works with local health
departments to develop a health education strategic plan and to develop a training plan to
build health education competency for local health departments throughout the state.
The total amount of this contract will be $24,430.34 with the addition of this amendment
for the contract period March 1, 1997 through February 28, 1998.
