RE: APPROVE RENEWAL LETTER#1 TO FAMILY PLANNING PROGRAM CONTRACT
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Renewal Letter#1 to the Family
Planning Program Contract between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 1999, and ending December 31, 1999, with further terms and
conditions being as stated in said renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Renewal Letter#1 to the Family Planning Program Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 7th day of July, A.D., 1999, nunc pro tunc July 1, 1999.

SUBJECT: Renewal Letter#1 to Family Planning Program Contract
Enclosed for Board review and approval is renewal letter#1 to the Family Planning Program
contract between Weld County Department of Public Health and Environment (WCDPHE) and
the Colorado Department of Public Health and Environment.
Under the terms of this renewal letter, WCDPHE will be reimbursed an amount not to exceed
$54,693 to provide contraceptive services to approximately 900 clients and$975 for cryosurgery
and/or pap smear screening services for 13 clients. This amounts to a total of$55,668 for the
period July 1, 1999 through December 31, 1999.
This program continues to be one of the tools we use in working with the teen pregnancy issue in
Weld County. I recommend your approval of this renewal letter.

Contract Renewal Letter No. 1
In accordance with Paragraph C4 of the contract with routing number 9900204 and contract number
FPP9900204, hereinafter referred to as the Original Contract(copy attached and by this reference made a part
hereof) between the state of Colorado,Department of Public Health and Environment, Family Planning
Program and Weld County Health Department, the parties hereby agree the Original Contract is hereby
renewed for the period July 1, 1999 through December 31, 1999. The parties agree that the maximum amount
payable by the State for the eligible services during this renewal period is Fifty-five thousand six hundred
sixty-eight Dollars($55.668 ). The state will pay Fifty-four thousand six hundred ninety-three Dollars
($54,693)to provide family planing services to QQQ clients and Nine hundred seventy-five dollars ($975)for
cryosurgery and/or pap smears screening services for j3 clients. The terms and conditions of the original
contract are hereby reaffirmed and will continue in full force and effect throughout this renewal period unless
otherwise stated.
This amendment to the Original Contract is intended to be effective July 1, 1999, but in no event will it be
deemed valid until it will have been approved by the State Controller or such assistant as he may designate.
