RE: APPROVE LETTER OF AGREEMENT WITH NORTHEASTERN COLORADO AREA
AGENCY ON AGING AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Letter of Agreement between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and
Northeastern Colorado Area Agency on Aging, commencing February 5, 1998, and ending
April 15, 1998, with further terms and conditions being as stated in said letter of agreement, and
WHEREAS, after review, the Board deems it advisable to approve said letter of
agreement, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Letter of Agreement between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Area Agency on Aging, and the Northeastern Colorado Area
Agency on Aging be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said letter of agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of February, A.D., 1998, nunc pro tunc February 5, 1998.

Dear Linda:
Please consider this an official Agreement between Weld County Area Agency on Aging and the Northeastern
Colorado Area Agency on Aging to contract the services of Eva Jewel, Weld County Single Entry
Point/Options for Long Term Care Supervisor, for training of 6 staff members on the Options for Long Term
Care case policies and procedures for operating the program (approximately 25-35 hours of training) at the
rate of$16.00 per hour. Training will begin February 5. Any related mileage incurred will be paid at the rate
of 27 cents per mile.

Subject. Agreement between the Weld County Area Agency on Aging and the
Northeastern Colorado Area Agency on Aging
Enclosed for Board approval is an Agreement between the Northeastern Colorado Area Agency
on Aging and the Weld County Area Agency on Aging. The Northeastern AAA is purchasing
training hours from the Weld County AAA to assist them in bringing their Options for Long Term
Care Program (case management) on line.
Weld County AAA will be reimbursed at $16.00 per hour and .27 cents per mile to provide
approximately 35 hours of training. Training will begin February 5, 1998, and be terminated by
April 15, 1998.
