EYEGLASS SERVICES CONTRACT
FOR THE YEAR, 1968:
WHEREAS, the Weld County Optometrists and Ophthalmologists Association
has submitted an eyeglass services contract for the year 1968 to the Board of
County Commissioners, copy of which is hereto attached and made a part of this
resolution, and
WHEREAS, after careful review and consideration, the Board believes it
to be in the best interests of the County to accept said contract.
NOW, THEREFORE, BE IT RESOLVED, by the Board of County Commissioners
of Weld County, Colorado that the eyeglass services contract as submitted by
the Weld County Optometrists and Ophthalmologists Association be and the same
is hereby accepted and signed.
The above and foregoing resolution was, on motion duly made and
seconded, adopted by the following vote:

Welfare Vision Care Servic
This contract executed at Greeley, Colorado this 5th day of January,1968
by and between the Board of County Commissioners of Weld County, Colorado hereinafter
called the County, and the Optometrists and Ophthalmologists of Weld County,
hereinafter called Association.
Witnesseth:
That in consideration of the mutual promises and obligations and other
considerations herein expressed, it is mutually agreed until December 31, 1968
that:
1. All eyeglass services shall be provided by prior authorization in
writing by the Weld County Department of Public Welfare.
2. Glasses are to be provided for recipients of Aid to Dependent Children,
children of General Assistance recipients, children in foster home care,
and for adults in cases of extreme necessity only.
3. The fee for examination is established at $10.00; the County to pay $9.00,
the patient to pay $1.00.
4. The cost of lenses is not be exceed $14.00 per pair for first division
hardened lenses and the frame is not to exceed $10.00 and will be based
on a thirty percent discount of preceding standard fees for Welfare approved
patients. A low price standard frame is to be provided.
5. Patient is to pay 107. of the complete vision analysis, lens and frame
service fees and material costs; minimum fee $1.00.
6. The exception to the above 107. fee to be paid by the patient is Welfare
children in foster care homes.
7. A review consisting of representatives of the Optometric and Ophthalmological
professions will be established to review the billings and to adjudicate any
unusual fee. This committee will furnish a semi-annual report of expenditures
in funds in detail to the County.
8. Only members of good standing in their respective and accredited society
and/or association will receive appointment as a participating member on
the Welfare fee basis program.
9. Total not to exceed $8,500.00 for the year, 1968.

PROPOSED BUDGET FOR WELD COUNTY
WELFARE VISION CARE SERVICES
FOR THE YEAR 1968
Program as apergLgq in 1967:
1. All eyeglasses and vision analysis services are provided only by prior
authorization of the Weld County Department of Public Welfare.
2. Vision analysis and glasses are provided for recipients of Aid to
Dependent Children assistance, children in foster care, borderline
assistance cases and adults in cases of extreme necessity.
3o The fee for examination is $10.00. The County pays $9.00; the patient
:pays $1.00.
4. The cost of lenses does not exceed $140OO per pair for first division
hardened lenses and the frame does not exceed $10.00 and is based
on a thirty percent discount of preceding standard fees for Welfare
approved patients. A low price standard frame is provided.
5. The patient pays 106/ of the complete vision analysis, lens and frame
service fees and material costs. The minimum fee paid by patient is
$1.00,
6. The exception to the above foe paid by the patient is Welfare
children in foster care homes.
7. A committee consisting of representatives of the optometric and
ophthalmological professions reviews the billing and adjudicates any
unusual fee. This committee furnishes a semi-annual report of
expenditure of funds to the County.
8. Only members of good standing in their respective and accredited society
and/or association are permitted to participate in the County Welfare
Vision Care Services program.
9. County allocation for 1967, $8i500.00.
Requested c a,u s Iox :
1. That the budget be increased to $12,000.00.
Reasons for requested budget increase:
1. $12,500.00 Was requested for 1967.
2. The amount requested is considered to be realistic as the present
budget only permits vision care services for approximately seven
months of 1967.
3. The present amount allocated is now sufficient to take care of vision
care needs of welfare children through the winter, spring, and part of
the fall school sessions. The program is inoperable during the summer
months and the latter part of the year because of lack of funds.

4. The early part of the school year is when children are in the greates
need of eye care. Sufficient funds do not remain to take care of
minimum needs during the traditionally busiest time of the year of
the eye care professions.
5• Recognizing the limited funds available, welfare case workers have '.een
authorizing eye care only when the need is obviously great.
6. The program has been limited almost completely to children. Less than
five percent of the recipients have been adults.
7. For vision rehabilitation and efficiency and resulting employment the
program should be made available to more needy adults.
8. Recipients of vision aid for 1967 are projected to total 350. In
view of the ensuing vision needs of school children and the uncared
for vision needs of indigent adults, it is estimated that a minimum
of 500 welfare patients will need vision assistance in 1968.
