RE: APPROVE TWO NOTIFICATIONS OF FINANCIAL ASSISTANCE AWARDS FOR
PLACEMENT ALTERNATIVES COMMISSION FUNDS UNDER THE WELD COUNTY
FAMILY SUPPORT PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with two Notifications of Financial
Assistance Awards for Placement Alternatives Commission Funds Under the Weld County
Family Support Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and the Colorado Department of Human Services, commencing October 15, 1997,
and ending September 30, 1998, with further terms and conditions being as stated in said
notifications, and
WHEREAS, after review, the Board deems it advisable to approve said notifications,
copies of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the two Notifications of
Financial Assistance Awards for Placement Alternatives Commission Funds Under the Weld
County Family Support Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Social Services, and the Colorado Department of Human Services be, and hereby are,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said notifications.

Weld County Family Support Pr'_ am
Enclosed for Board approval are FY 97-98 Notification of Financial Assistance Awards (NOFAA)
for Family Preservation and Family Support Funds for FY 1997-98, beginning October 15, 1997,
and ending September 30, 1998.The Contract between the County of Weld, State of Colorado, by
and through the Weld County Board of Commissioners, on behalf of the Department of Social
Services, and the Colorado Department of Human Services was approved by the Board on
December 24, 1997.
The contracted providers for the Weld County Family Support Program are
1) Catholic Charities:
a) Funding level is $90,000 with a match of$10,000, to be provided by the Contractor.
b) Catholic Charities will serve 142 at-risk families in the Greeley area and South Weld
County.
2) Weld County Department of Human Services:
a) Funding level is $89,000 with a match of$10,000, to be provided by the Contractor.
b) The Department of Human Services will serve 75 at-risk families in the Greeley area
and South Weld County.
