HomeMy WebLinkAbout961227.tiff RESOLUTION RE: APPROVE TWO PURCHASE OF SERVICES AGREEMENT AMENDMENTS BETWEEN AREA AGENCY ON AGING AND HEALTH DEPARTMENT NURSING DIVISION AND CHARLES J. CONNELL, ATTORNEY AT LAW, AND AUTHORIZE CHAIRMAN TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with two Purchase of Services Agreement Amendments between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Services Area Agency on Aging, and the Weld County Health Department Nursing Division and Charles J. Connell, Attorney at Law, commencing June 1, 1996, and ending December 31, 1996, with further terms and conditions being as stated in said agreement amendments, and WHEREAS, after review, the Board deems it advisable to approve said agreement amendments, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the two Purchase of Services Agreement Amendments between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division of Human Services Area Agency on Aging and the Weld County Health Department Nursing Division and Charles J. Connell, Attorney at Law, be, and hereby are, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said agreement amendments. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 17th day of July, A.D., 1996, nunc pro tunc June 1, 1996. Webster 961227 (1' : flS; lilt)AK f/L ;flenl)el/ HR0067 PURCHASE OF SERVICES AGREEMENT AMENDMENT THIS AMENDMENT, made and entered into this 15th day of July, 1996 by and between the Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA," and Weld County Health Department's Nursing Division,hereinafter referred to as "WCHD," WITNESSETH: WHEREAS, the "AAA" desires to continue to purchase health promotion/disease prevention services for WCHD for older persons at risk of declining health due to isolation and irregular monitoring of health status, WHEREAS, WCHD is a Weld County Governmental organization existing for the purpose of providing health education, health screening,and in home case management, NOW THEREFORE, it is mutually agreed: 1. The "AAA" will extend the term of the original agreement to December 31, 1996. 2. The "AAA" will reimburse "WCHD" up to $3,500 for the time period of June, 1996 through December 31, 1996 for public nurse time for two rural well adult clinics at$16.00 per hour for 3 hours per month per site and for public nurse time at$16.00 per hour for in home health case management. 3. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. 4. This Amendment will be terminated immediately in the event of loss of funding. IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the day first written above. Connell, Attorney at Law, hereinafter referred to as " CONNELL." WITNESSETH: WHEREAS, the "AAA" desires to continue to purchase legal services from " CONNELL", and WHEREAS, the "AAA" and "CONNELL" are desirous of amending the original agreement of January 2, 1995, NOW THEREFORE, it is mutually agreed: 1. The "AAA" will reimburse "CONNELL" $7,500 for legal consultation and representation for the "AAA" Senior Legal Aide Program. 2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. 3. The terms of this Amendment extend from January 2, 1996 through December 31, 1996. This Amendment will be terminated immediately in the event of loss of funding. Speckman, Executive Director Human Services Subject: Area Agency on Aging Purchase of Service Agreement Amendments Date: July 9, 1996 Enclosed for Board approval are amendments to purchase of service agreements between the Area Agency on Aging and the Weld County Health Department and the Area Agency on Aging and Charles Connell, Attorney at Law. The Weld County Health Department Agreement has been extended to end December 31, 1996 with an agreement to reimburse the Health Department $3,500 for the time period of June 1, 1996 through December 31, 1996. This agreement provides for health monitoring of isolated, at risk elders as well as support for Well Adult Clinics at designated senior center sites. The agreement with Charles Connell is amended to increase the reimbursement from $5,000 to $7,500 to cover legal service needs for seniors not anticipated at the time of the original agreement.
