This Notice is tendered pursuant to the
"Colorado Governmental Immunity Act, " C.R. S . 1973 ,
24-10-109 . Thank you in advance for your prompt
attention and concern.
1 . NAME AND ADDRESS OF CLAIMANT. The name and
address of the claimant is U S WEST
Communications, Area Claims Office, 1801
California, Ste 1130 , Denver, Colorado 80202 .
2 . STATEMENT OF BASIS OF CLAIM. On or about
04/24/95 , in the vicinity of ON WCR 39 .BETWEEN
CRS 38&40 , in the City of LA SALLE, and the State
of Colorado; one or more of your employees or
agents acting within the scope of their
employment damaged a 25 PAIR BURIED CABLE owned
by the claimant . Your employees or agents caused
the damage during CULVERT EXCAVATION AND A LOCATE
WAS NOT REQESTED.
3 . STATEMENT OF NATURE AND EXTENT OF INJURY. As a
result of the above described damage, the
claimant has sustained financial damage as a
result of having to employ its men and machinery
to repair the damage .
4 . AMOUNT OF DAMAGE . The amount of monetary damage
is $546 . 03 . Should you have any questions or
disagree with our assessment of damages, please
contact the undersigned.
