RE: APPROVE PREVENTIVE HEALTH AND HEALTH SERVICES BLOCK GRANT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Preventive Health and Health
Services Block Grant between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Health Department,
and the Colorado Department of Public Health and Environment, commencing March 1, 1999,
and ending February 28, 2000, with further terms and conditions being as stated in said grant,
and
WHEREAS, after review, the Board deems it advisable to approve said grant, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Preventive Health and Health Services Block Grant between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department, and the Colorado Department
of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said grant.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 29th day of June, A.D., 1998.

Public Health Leadership and Professional Development Training

1. Health People 2000 Objective to be addressed:
8.14- Increase to at least 90 percent the proportion of people who are served by a local health department
that is effectively carrying out the core functions of public health; assessment,policy development,and assurance.
2. Statement of Problem:
Strategies such as leadership development and health promotion through education increase the capacity of
health professionals and local public health agencies in Colorado to develop and disseminate practices that prevent
disease and disability. Individuals from various agencies,public health units,and professional groups in Colorado
have requested ongoing training to enhance leadership and professional development skills needed for community
health education and prevention programs.Quality training is not always accessible or affordable to staff at the local
public health agencies, such as nurses,environmental health specialists and allied health professionals
3. Program Strategy
Through the Health Education Standards Project, funded with Preventive Health and Health Services
Block Grant dollars, an infrastructure was developed to provide training based on regionally assessed needs for the
local public health agencies throughout Colorado. Cooperation from the local public health agencies and the
Colorado Department of Public Health& Environment has facilitated the successful implementation of 15 regional
workshops based on assessed needs. Ten additional workshops are planned for the remainder of the year and an
evaluation system is in place to support future training efforts. Alliances with AMC Cancer Research Center,
Connecting Colorado,University of Northern Colorado,University of Colorado Health Sciences Center have been
established to support the training process.As an adjunct to the Health Education Standards project,the Public
Health Leadership Series provides additional professional development opportunities for members of the Colorado
Environmental Health Association(CEHA),Colorado County Nurses Association(CCNA), and the Colorado
Public Health Association(CPHA).
The strategies in this project will build on the infrastructure developed by the Health Education Standards Project as
follows:
• Establish a multidisciplinary advisory group which includes representatives from local public health
agencies,professional groups,partner agencies/projects and the Colorado Department of Public Health&
Environment.
• Expand the scope of the project to include professional technical skills, leadership development,health
education and promotion, community development and build community capacity.
• Maintain and build on the present alliances with statewide organizations,professional organizations,
projects and agencies to maximize marketing capabilities, prevent duplication of training and offer
additional or advanced courses.
• Provide multi level,affordable training utilizing distance teaming capabilities and local and regional
workshops to reach all areas of Colorado.

6. Target Population:
All public health professionals in the 63 Colorado counties providing core public health functions,members of
Colorado Public Health Association, Colorado County Nurses Association, Colorado Environmental Health Association.
7. Disparate Population
This project will ultimately benefit all citizens of Colorado.
S. Impact Objectives:
• Timely training for public health professional in all areas of Colorado.
• Increase skills needed to provide quality core public health functions.
• Ultimately develop a quality model for training that can be replicated for future training for all local public health
agencies and organizations.
9. Annual Objectives:
• Increase utilization of the Resource Rolodex Web site to provide a network for technical assistance and expertise in
public health.
• Conduct a current training needs assessment with local public health agencies and professional organizations.
Utilize current assessment,utilization,and evaluation data to guide the planning process.
• Provide regional training based on previously identified needs concurrently with conducting the needs assessment
with local agencies and professional organizations.
•
Expand the Public Health Leadership development training, building on the present model, in collaboration with
the Colorado Department of Public Health&Environment and CPHA,CCNA and CEHA.
• Increase collaboration and participation in training offered by other agencies,coalitions and projects supporting
marketing efforts and utilizing existing web site and newsletters.

Subject: Preventive Block Grant Proposal for Pu is Health Leadership and
Professional Development Training
Enclosed for Board review and approval is a proposal to the Colorado Department of Public
Health and Environment to receive funding from the Preventive Block Grant to provide Public
Health Leadership and Professional Development Training.
This grant, if funded, will allow the Health Department to hire 1 FTE Health Eduction Specialist
to develop, coordinate, and evaluate training for public health professionals across the state. The
training will provide public health professionals with the knowledge and skills needed to
implement community health education and prevention programs. The funds will allow low cost
training programs to be brought to different regions of the state, allowing greater accessibility for
staff in rural areas of Colorado.
For these services, Weld County Health Department is requesting$63,285.91. The grant period
would be March 1, 1999 through February 28, 2000.
