HomeMy WebLinkAbout982029.tiff RESOLUTION RE: APPROVE MEMORANDUM OF AGREEMENT FOR FAMILY PLANNING PROGRAM PEER INTERVENTION PROJECT AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Memorandum of Agreement for the Family Planning Program Peer Intervention Project between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Health Department, and the Colorado Department of Public Health and Environment, commencing August 3, 1998, and ending June 30, 1999, with further terms and conditions being as stated in said agreement, and WHEREAS, after review, the Board deems it advisable to approve said agreement, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Memorandum of Agreement for the Family Planning Program Peer Intervention Project between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Health Department, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said agreement. The above and foregoing Resolution was, on motion duly made and seconded, adopted by the following vote on the 21st day of October, A.D., 1998, nunc pro tunc August 3, 1998. The funding will be used to train four additional peer educators and to provide additional contacts with low income and high risk adolescents in a variety of setting. The agency will maintain statistical information for project assessment as outlined in the proposal. Funding for the project begins August 3, 1998 and ends June 30, 1999. Billing will be submitted • for$3,750 at the February 26, 1999 midpoint and at the June 30, 1999 completion date for the remaining $3,750 unless other arrangements arc made. A midpoint project update is due February 26, 1999 and a Final report is due July 30, 1999. Harbert, Chair To Board of County Commissioners Date October 16, 1998 COLORADO O/ 2 p From John Pickle, Director, Health Department �+ Subject: Memorandum of Understanding for Peer Intervention Project Enclosed for Board review and approval is a memorandum of understanding between Weld County Health Department and the Colorado Department of Public Health and Environment for the Family Planning Program Peer Intervention Project. The Health Department has been awarded continuation funding in the amount of$7,500 for this project for the period August 3, 1998 through June 30, 1999. The purpose of the project is teen pregnancy prevention using peer-based education and support to high-risk youth in both one-on- one and group settings. The peer education is comprehensive to not only include facts on teen pregnancy, abstinence and other birth control options, but also HIV/STDs, date rape/sexual assault, communication and negotiation skills, self-esteem, and community resources. The$7,500 will provide for five hours per week of a health educator's time to coordinate the program and to pay a stipend to four trained peer educators. Educational materials are also included in the budget for this program. I recommend your approval of this memorandum of understanding. Enclosure 982029 Hello
