RE: APPROVE CONTRACT RENEWAL LETTER #1 FOR CHLAMYDIA TESTING
CONTRACT AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Contract Renewal Letter#1 for the
Chlamydia Testing Contract between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Health
Department, and the Colorado Department of Public Health and Environment, commencing
January 1, 1999, and ending December 31, 1999, with further terms and conditions being as
stated in said renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Contract Renewal Letter#1 for the Chlamydia Testing Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Health Department, and the
Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 13th day of January, A.D., 1999.

Subject: Renewal Letter#1 to Chlamydia Testing Contract
Enclosed for Board review and approval is Renewal Letter#1 to the Chlamydia Testing Contract
between Weld County Health Department (WCHD) and the Colorado Department of Public
Health and Environment.
This renewal letter provides for Weld County to be reimbursed for chlamydia tests done in the
sexually transmitted disease and family planning clinics. The reimbursement rate is $7.80 per
test not to exceed a total amount of$5,226 for the period January 1, 1999 through December 31,
1999.
I recommend your approval of this renewal letter.

Contract Renewal Letter No. 1
Pursuant to paragraph 2 of the contract with routing number 97-06275 and contract encumbrance number
EPI9706275, (as amended by Contract Amendment No. 1, routing number 98-07899, and Change Order Letter No.
I, routing number 99 FHA 00030), hereinafter referred to as the"Original Contract"(a copy of which is attached
hereto and by this reference made a part hereof) between the State of Colorado,Department of Public Health and
Environment and Weld County Health Department, for the renewal term from January 1 , 1999 through
December 31 , 1999,the parties agree that the maximum amount payable by the State for the eligible services
referenced in paragraph 2 of the Original Contract is increased by Five Thousand Two Hundred Twenty Six
Dollars(55,226.00) according to the budget outlined below, to a new total of Fourteen Thousand One Hundred
Twenty One Dollars($14,121.00). The first sentence in paragraph 2 of the Original Contract is hereby modified
accordingly. All other terms and conditions of the Original Contract are hereby reaffirmed.
Chlamydia Testing in Family Planning and STD Clinics
Januaryl, 1999 - December 31, 1999
a. Family Planning Clinic Chlamydia Tests
(550 tests @ $7.80 each) $ 4,290.00
b. STD Clinic Chlamydia Tests
Female and male contacts
(120 tests @ $7.80 each) $ 936.00
TOTAL: $ 5,226.00
This amendment to the Original Contract is intended to be effective as of January 1, 1999, but in no event shall it be
deemed valid until it shall have been approved by the State Controller or such assistant as he may designate.
