RE: APPROVE AMENDMENT TO AGREEMENT FOR WELD COUNTY SHERIFF'S
OFFICE DETENTION CENTER MEDICAL UNIT SERVICE AND AUTHORIZE CHAIR
TO SIGN - PREFERRED CORRECTIONAL MEDICAL TEAM, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to Agreement for Weld
County Sheriff's Office Detention Center Medical Unit Service between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Sheriff, and Preferred Correctional Medical Team, Inc., commencing
January 1, 1998, and ending December 31, 1998, with further terms and conditions being as
stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to Agreement for Weld County Sheriffs Office
Detention Center Medical Unit Service between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Sheriff, and Preferred Correctional Medical Team, Inc. be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 9th day of March, A.D., 1998, nunc pro tunc January 1, 1998.

AMENDMENT TO
AGREEMENT FOR WELD COUNTY SHERIFF'S OFFICE
DETENTION CENTER MEDICAL UNIT SERVICE
THIS AMENDMENT, dated for reference purposes only, the 31st day of December, 1997, is made
and entered into by and between the board of County Commissioners of Weld County, Colorado,
("County") and Preferred Correctional Medical Team, Inc. ("PCMT"). It is hereby attached to and
shall become a part of the Agreement by and between the County and PCMT which was executed
September 11, 1988.
WITNESSETH:
WHEREAS, the County and PCMT entered into a Medical Unit Service Agreement ("Agreement")
September 11, 1988; and
WHEREAS, said Agreement has been renewed and extended by Amendments through December
31, 1997; and
WHEREAS, pursuant to Paragraphs VI and XVIII of the Agreement, the parties may renegotiate
the payment provisions of the Agreement prior to the commencement of the new term for the
following twelve (12)months; and
WHEREAS, the Sheriff's Office of Weld County desires the Agreement, which now covers both
the Centennial and the North Complex facilities, be renewed and has negotiated the following
changes, subject to approval of the Board of County Commissioners.
NOW, THEREFORE, for good and valuable consideration, the receipt and sufficiency of which are
hereby acknowledged, County and PCMT agree to the following:
1. The term of the Agreement is hereby renewed and shall commence January 1, 1998, and
continue to and including December 31, 1998.
2. The monthly payment of$46,800.00 shall be paid and adjusted as follows:
a. For the months of January, February, and March 1998, the monthly payment shall
remain $46,800.00; and
b. For the months of April through December 1998, the monthly payment shall be
$54,800.00
3. The Agreement, as amended hereby, shall remain in full force and effect through December
31, 1998.
