RE: APPROVE CHANGE ORDER LETTER FOR SINGLE ENTRY POINT CONTRACT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Change Order Letter for the Single
Entry Point Contract between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services,
Area Agency on Aging, and the Colorado Department of Health Care Policy and Financing, with
terms and conditions being as stated in said change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said change order
letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Change Order Letter for the Single Entry Point Contract
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the Colorado Department of Health Care Policy and Financing be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 5th day of May, A.D., 1999.

SUBJECT: Amendment to the Single Entry Point, Weld Area
Agency on Aging
Attached are five (5) copies of the Amendment to the current Single
Entry Point contract . The overall contract amount will be
increased from $338, 552 to $348, 608 due to the increase of clients
being served by the Single Entry Point . The change to the contract
will be effective May 10, 1999 .

In accordance with Paragraph 10 of Page 2 of Exhibit A of contract routing number 3398-1323 between the State of
Colorado Department of Health Care Policy and Financing and Weld County Board of Commissioners by and
through the Weld County Division of Human Services' Area Agency on Aging, 1551 North 17th Avenue Greeley,
CO 80631covering the period of July 1, 1998 through June 30, 1999 the undersigned agree the first sentence of
Paragraph 9, Page 7, Compensation/Maximum Payable is hereby amended to read:
Payment pursuant to this Contract will be made as earned, in whole or in part, from available state
funds encumbered in an amount not to exceed$348,608 for the purpose of the within described
services.
This constitutes a(n) Increase in the maximum compensation to be made to the Contractor by the State under this
contract of$10,056.
The sentences on Page 2 of Exhibit A to the contract which now read:
However, total compensation for the contract term shall not be increased under this mechanism by
more than$35,196 above the base contract amount, i.e., to a maximum total compensation of
$338,552. The maximum increase above the base contract amount represents 42 clients above the
base client figure; the figure of 404 shall be known as the "maximum reimbursable client figure".

more than$45252 above the base contract amount, i.e., to a maximum total compensation of
$348,608. The maximum increase above the base contract amount represents 54 clients above the
base client figure; the figure of 416 shall be known as the "maximum reimbursable client figure".
This amendment to the contract is intended to be effective as of May 10, 1999, but in no event shall it be deemed
valid until it shall have been approved by the State Controller or such assistant as he may designate.
