RE: APPROVE RENEWAL LETTER #2 FOR STATE PER CAPITA CONTRACT AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Renewal Letter#2 for the State Per
Capita Contract between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing October 1, 1999, and ending June 30, 2000, with further terms and conditions
being as stated in said renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Renewal Letter#2 for the State Per Capita Contract between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
the Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of September, A.D., 1999.

SUBJECT: State Per Capita Contract Renewal Letter#2
DATE: September 14, 1999
Enclosed for Board review and approval is the State Per Capita Contract Renewal Letter No. 2
between the Colorado and Weld County Departments of Public Health and Environment.
The State per capita appropriation for Weld County Department of Public Health and
Environment for the time period October 1, 1999 through June 30, 2000 is $209,650. This is
$18,508 more than we received last year for local public health efforts.
I recommend your approval of this renewal letter.

September 30, 1999 Contract Renewal Letter
State Fiscal Year 1999-00, Contract Renewal Letter Number 2, Contract Routing Number 00-00730
Pursuant to paragraph#7 of the contract with contract routing number 98-7021 and contract encumbrance number
ADM9807021, (as amended by Contract Renewal Letter 2 contract routing number 00-00730 hereinafter referred to as
the"Original Contract"(a copy of which is attached hereto and by this reference incorporated herein and made a part
hereof)between the State of Colorado,Department of Public Health and Environment and Weld County Health
Department,for the renewal term from October 1.1999 through June 30.2000,the parties agree that the maximum
amount payable by the State for the eligible services referenced in paragraph 7 of the Original Contract is increased by
TWO HUNDRED AND NINE THOUSAND.SIX HUNDRED FIFTY DOLLARS ($209,650.00)for a new total
financial obligation of the State of FIVE HUNDRED EIGHTY-TWO THOUSAND.ONE HUNDRED FORTY-TWO
DOLLARS ($582,142.00) . The revised budget,which is attached hereto as"Attachment 1",is incorporated herein by
this reference and made a part hereof. The first sentence in paragraph 7 of the Original Contract is hereby modified
accordingly. All other terms and conditions of the Original Contract are hereby reaffirmed. This amendment to the
Original Contract is intended to be effective as of October 1.1999. However,in no event shall this amendment be deemed
valid until it shall have been approved by the State Controller or such assistant as he may designate.
Please sign,date,and return all 4 originals of this Contract Renewal Letter as soon as possible to the attention of Julia
Akin,Colorado Department of Public Health and Environment,4300 Cherry Creek Drive South,Denver,Colorado

approved.
