RE: APPROVE PURCHASE OF SERVICES AGREEMENT AMENDMENT AND
AUTHORIZE CHAIR TO SIGN - MEALS ON WHEELS, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement
Amendment between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Department of Human Services, and
Meals on Wheels, Inc., with terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement Amendment between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, and Meals on Wheels, Inc., be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 22nd day of July, A.D., 1998.

COLORADO SUBJECT: Meals on Wheels Agreement Amendment
Enclosed for Board approval is a purchase of service agreement amendment with Meals on Wheels
increasing the FY 1998 award from $26,885 to $30,885. Meals on Wheels agrees to serve 24,650
meals to 125 homebound elderly through December 31, 1998. The amendment is intended to assist
Meals on Wheels with increasing demands for home delivered meals for frail elders.
All other terms of the FY 1998 agreement remain unchanged.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 15th day of April. 1998 by and between the
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as
"AAA," and Meals on Wheels, Inc., hereinafter referred to as "MOW."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home delivered meals from "MOW", and
WHEREAS, the "AAA" and "MOW" are desirous of amending the original agreement of
January 2, 1995,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse " MOW" $30,885 for 24,650 home delivered meals for 125 persons,
at least 20 to be minority.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise
stated in this Amendment.
3. The terms of this Amendment extend from July 15, 1998 through December 31, 1998. This
Amendment will be terminated immediately in the event of loss of funding.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the day first written
above.
