RE: APPROVE ADDENDUM TO PURCHASE OF SERVICES AGREEMENT BETWEEN
THE WELD COUNTY HEALTH DEPARTMENT AND THE UNIVERSITY OF
NORTHERN COLORADO SPEECH -LANGUAGE PATHOLOGY AND AUDIOLOGY
CLINIC AND AUTHORIZE CHAIRMAN TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Addendum to Purchase of Services
Agreement between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Health Department and
the University of Northern Colorado Speech -Language Pathology and Audiology Clinic,
commencing January 1, 1997, and ending June 30, 1997, with further terms and conditions
being as stated in said addendum, and
WHEREAS, after review, the Board deems it advisable to approve said addendum, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Addendum to Purchase of Services Agreement between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Health Department and the University of Northern
Colorado Speech -Language Pathology and Audiology Clinic be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said addendum.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of February, A.D., 1997, nunc pro tunc January 1, 1997.

ADDENDUM TO PURCHASE OF SERVICES AGREEMENT
I. Child Health Clinic
A. Audiological testing will be provided at the Weld
County Health Department on the following dates:
1. January 23, 1997
2. February 27, 1997
3. March 27, 1997
4. April 24, 1997
5. May 22, 1997
6. June 26, 1997
II. Well Adult Clinics
A. Audiological testing will be provided at the Weld
County Health Department on the following dates:
1. January - No Clinic
2. February 19, 1997
3. March 12, 1997
4. April 9, 1997
5. May 14, 1997
6. June 11, 1997
III. Provide one inservice on March 27, 1997, to Weld County
Health Department nurses regarding hearing loss and
audiological and hearing aid services in Weld County and
procedures to access these services.
IV. Payment for the above services shall be:
$2362.00 for clinics
$ 50.00 for the inservice
Total $2412.00, which will be billed June 1, 1997.

Subject: Addendum to Purchase of Services Agreement with UNC Speech-
Lanauaae Pathologv and Audiologv Clinic
Enclosed for Board review is an addendum to the Purchase of Services Agreement
between Weld County Health Department (WCHD) and the University of Northern
Colorado (UNC) Speech -Language Pathology and Audiology Clinic.
This addendum sets the schedule for UNC to provide audiological testing services at both
the Child Health and Well Adult Program Clinics in the Health Department building for
the time period January 1, 1997 through June 30, 1997. Also, UNC will provide one
inservice to WCHD nurses regarding hearing loss and audiological and hearing aid
services available in Weld County. For these services, WCHD will pay UNC $2,412.00.
The schedule is only for the first half of 1997 because the sound booth which is currently
located in the WCHD building will be moved to the Monfort Children's Clinic at the end
of June. This booth belongs to the UNC Speech -Language Pathology and Audiology
Department, and it was determined with the increased number of clients at the Monfort
Children's Clinic there would be a larger need for these services at that site.
I recommend your approval of this addendum.
