Home My WebLink About980048.tiff RESOLUTION RE: APPROVE SEVEN PURCHASE OF SERVICES AGREEMENT AMENDMENTS WITH VARIOUS SERVICE PROVIDERS AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Seven Purchase of Services Agreement Amendments between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and the following services providers: 1. Meals on Wheels 2. Catholic Charities Northern 3. Catholic Charities Northern 4. Eldergarden 5. North Range Behavioral Health 6. Weld County Health Department 7. Charles J. Connell, Attorney at Law WHEREAS, said amendments commence January 2, 1998, and end December 31, 1998, with further terms and conditions being as stated in said amendments, and WHEREAS, after review, the Board deems it advisable to approve said amendments, copies of which are attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Seven Purchase of Services Agreement Amendments between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency on Aging, and Meals on Wheels, Catholic Charities Northern, Catholic Charities Northern, Eldergarden, North Range Behavioral Health, Weld County Health Department, and Charles J. Connell, Attorney at Law, be, and hereby are, approved. WITNESSETH: WHEREAS, the "AAA" desires to continue to purchase home delivered meals from "MOW", and WHEREAS, the "AAA" and "MOW" are desirous of amending the original agreement of January 2, 1995, NOW THEREFORE, it is mutually agreed: 1. The "AAA" will reimburse " MOW" $26,885 for 24,650 home delivered meals for 125 persons, at least 20 to be minority. 2. All other terms and conditions of the original agreement shall remain in effect unless otherwise stated in this Amendment. 3. The terms of this Amendment extend from January 2, 1998 through December 31, 1998. This Amendment will be terminated immediately in the event of loss of funding. The Purchase of Services Agreements are between the Weld County Division of Human Services' Area Agency on Aging and the following agencies: 1.
