RESOLUTION
RE: APPROVE FIRST AMENDMENT TO PURCHASE OF SERVICES AGREEMENT WITH
NORTHEAST COLORADO HEALTH DEPARTMENT AND AUTHORIZE CHAIR TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a First Amendment to Purchase of
Services Agreement between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Health Department, and the
Northeast Colorado Health Department, commencing January 7, 1998, and ending March 31,
1998, with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the First Amendment to Purchase of Services Agreement between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Health Department, and the Northeast Colorado Health
Department be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 7th day of January, A.D., 1998.

FIRST AMENDMENT TO PURCHASE OF SERVICES AGREEMENT
THIS FIRST AMENDMENT to Purchase of Services Agreement is made and entered
into this f day of (?-2:24z62(?-2:24z624-5,- , 19r, by and between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Health Department("WCHD"), and the Northeast Colorado Health
Department ("NCHD").
WITNESSETH:
WHEREAS, the above stated parties entered into a Purchase of Services Agreement for
the term from April 1, 1997 through March 31, 1998, which provided for case management and
outreach services to persons infected or affected by HIV in Morgan, Logan, Yuma, Sedgwick,
Phillips, and Washington Counties ("the Original Agreement"); and
WHEREAS, Paragraph 11 of the Original Agreement provides that any modification or
amendment of the Original Agreement must be mutually agreed upon in writing by the parties to
the Original Agreement; and
WHEREAS, WCHD has been awarded additional funding for the current year from the
Ryan White C.A.R.E. Title II funds from the Colorado Department of Public Health and
Environment for use in establishing a consortium for care services for persons infected and
affected by the HIV virus in Weld, Larimer, Morgan, Logan, Yuma, Sedgwick, Phillips, and
Washington Counties ("the Program"); and
WHEREAS, WCHD desires to amend the Original Agreement with NCHD to provide for
additional case management and other support services for a targeted population of migrant and
Hispanic/Latino populations of Northern Colorado; and
WHEREAS, NCHD desires to provide the additional case management and other support
services as above stated; and
WHEREAS, the parties desire to reduce the terms of their amendment to the Original
Agreement to writing.
NOW THEREFORE, for and in consideration of the covenants, conditions, agreements,
and stipulations hereinafter expressed, the parties do hereby amend the Original Agreement as
follows:
A. The above recitals are hereby incorporated herein as if stated verbatim.
B. The first subparagraph of Paragraph 3 of the Original Agreement is amended as
follows:

In consideration of the services to be provided by NCHD as set forth in this
Agreement, WCHD agrees to pay NCHD on a monthly basis for services
provided during that month, pursuant to the terms of this Agreement, and
the budget amounts set forth in Attachment 1, attached hereto and
incorporated herein.
The remaining subparagraph included in Paragraph 3, which begins "NCHD shall
submit an invoice on a form as set forth in Attachment B..." is not amended and
shall remain the same.
C. Paragraph 4 A. is amended to add the following to the existing paragraph:
NCHD shall also provide case management and other support services,
specifically serving the target population of migrant and Hispanic/Latino
populations of Northern Colorado. NCHD will coordinate its activities with
the Northern Colorado Health Network, Inc., dba the Northern Colorado
AIDS Project, to ensure appropriate provision of services and to eliminate as
much as possible any duplication of services.
D. The balance and remainder of the Original Agreement shall continue and remain
in effect with no amendment.
E. This Amendment to the Original Agreement shall become effective upon signing
by all of the parties, and shall continue through March 31, 1998.

above set forth.

Subject: Amendment to Purchase of Services Agreement with Northeast
Colorado Health Department
Enclosed for Board review and approval is an amendment to the purchase of services agreement
between Weld County Health Department and the Northeast Colorado Health Department
(NECHD).
Weld County Health Department recently received an additional award of$30,523 from the
Colorado Department of Public Health and Environment for the Ryan White Grant. This money
was specifically designated to be passed through to NECHD and Northern Colorado AIDS
Project (NCAP) for case management services for persons infected and affected by HIV in
Larimer, Morgan, Logan, Yuma, Sedgwick, Phillips, and Washington Counties.
This amendment provides additional funding to NECHD in the amount of$3,999.40 for that
agency to provide additional services in Morgan, Logan, Yuma, Sedgwick, Phillips, and
Washington Counties. The term of the amendment is from the date of its signing through March
31, 1998.
