AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with an Addendum to Child Protection Agreement between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and the Island Grove Regional Treatment Center, Inc., commencing February 1, 1998, and ending May 31, 1998, with further terms and conditions being as stated in said addendum, and WHEREAS, after review, the Board deems it advisable to approve said addendum, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, ex-officio Board of Social Services, that the Addendum to Child Protection Agreement between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Social Services, and the Island Grove Regional Treatment Center, Inc., be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said addendum. FY97-3003 CHILD PROTECTION AGREEMENT FOR SERVICES BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND THE ISLAND GROVE REGIONAL TREATMENT CENTER, INC. This Addendum to Contract No. FY 97-3003, made and entered into the first day of February 1998, by and between the Board of Weld County Commissioners on behalf of the Weld County Department of Social Services,hereafter referred to as"Social Services": and the Island Grove Regional Treatment Center, Inc.,hereinafter referred to as"Island Grove." WITNESSETH WHEREAS, required approval, clearance, and coordination have been accomplished from and with appropriate agencies; and WHEREAS, the Child Welfare Settlement Agreement required, among other things, Social Services to obtain outpatient core services for eligible clients who are at imminent risk for out-of-home placement in the category of alcohol and drug services; and WHEREAS,the Colorado Department of Human Services has provided Family Issues Cash Fund resources to Social Services for outpatient core services for families, children, and adolescents; and WHEREAS, Social Services and Island Grove desire to enter into an agreement in providing expanded services for families, children, and adolescents utilizing the above-mentioned resources, NOW THEREFORE, in consideration of the premises, the parties hereto covenant and address the following addendum to the Child Protection Agreement for Services. 1. Term This addendum shall become effective February 1, 1998, upon proper executive of this amended Agreement and shall expire May 31, 1998. 2. Scope of Services Island Grove shall place a CAC II or above on site at Social Services for 20 hours per week for the purpose of accompanying Social Service staff on follow-up visits to perform a behavioral health screening for the purpose of identifying clients who could benefit from further assessments for alcohol/drug intervention or mental health interventions. In addition,this on-site staff will perform alcohol and drug assessments as time permits, provide case consultation, case management, and in-house training on pertinent alcohol/drug issues as requested. 980290 ADDENDUM TO CONTRACT NO. FY97-3003 Child Protection Agreement for Services Between the Weld County Department of Social Services and the Island Grove Regional Treatment Center, Inc. The on-site staff will also assist in the development of their function as part of the EPP program which Social Services shall be initiating in the immediate future. The on-site staff shall accompany Social Service staff on all follow-up visits in order to perform an initial screening for alcohol/drug or mental health issues within family members. If the initial screening indicates that further assessment is needed in either area, the on-site staff person will make that recommendation to the caseworker. The caseworker would make the referral for either an alcohol/drug assessment or mental health assessment. If a mental health assessment is determined to be necessary, the caseworker shall refer to the appropriate mental health agency. If an alcohol/drug assessment is requested,the caseworker shall refer to the on-site staff person. The on-site staff person shall complete the assessment within three work days, if time does not allow for this assessment to be completed in the time period (during the allocated 20 hours on site),they will make a referral to Island Grove for the formal assessment. A written report shall be completed within five work days of completing the assessment. The primary purpose of the on-site person shall be to accompany social services staff on follow-up visits for the purpose of screening for behavioral health issues and perform their function within the EPP program. On-site case consultation, formal assessments, and in-house training are secondary to their primary function. The outcomes of this pilot would be to identify clients who could be placed into an intervention program(Welfare Diversion)which could prevent further action on the Department of Social Services and lead to earlier identification of issues relating to the initial complaint,which if addressed, could result in expedited successful closure of the case. The measurements for these outcomes would be the number of families identified as benefiting from formal assessments, number of families completing alcohol and drug assessments, number of families completing the intervention/brief therapy recommendations, and the shortened length of time a case is open in the Social Service system as a result of this intervention. Clients referred into the intervention program would be considered voluntary, Social Services would keep the case open until at least eight hours of intervention services had been performed. At that time, the caseworker of record would be notified by Island Grove that the family is cooperative and has completed a minimum of eight hours of intervention services. It would then be the caseworker's option to close or continue to monitor the family, based on individual case parameters. Island Grove would continue to provide treatment services to the referred family until it is clinically appropriate to discharge, at that time a written discharge summary will be sent to the caseworker for placement in the file. 2 980230 ADDENDUM TO CONTRACT NO. FY97-3003 Child Protection Agreement for Services Between the Weld County Department of Social Services and the Island Grove Regional Treatment Center, Inc. Island Grove will pay for intervention treatment services after the initial referral is made on the clients referred through the on-site staff person. Social Services will benefit by having access to funds outside of their budget to provide treatment services to their clients. Social Services will pay for the on-site person out of the ADAD core services dollars and assessments which cannot be completed by the on-site staff person per the terms of the current contract. 3. Payment Social Services will pay a monthly fee to Island Grove for placing an on-site CAC staff in their facility for 20 hours per week. The fee covers Island Grove's costs as it relates to salary, benefits, taxes, clinical supervision of assigned personnel, supplies, travel and indirect costs associated with the pilot project. The fee will be$55 per hour and will be billed monthly through our current core services contract as on-site staff services. That would equate to $18,700 over the 17-week pilot period from February 1, 1998, through May 31, 1998. Except as modified herein, the terms and conditions of the original agreement FY97-3003, together with all attachments of the original Agreement, remain in full force and effect. The major provisions of the addendum are as follows: 1. The term of the addendum will be from February 1, 1998 through May 31, 1998. 2. The Weld County Department of Social Services and the Island Grove Regional Treatment Center wishes to enter a 17-week pilot. Island Grove Regional Treatment Center agrees to place one of their staff(CACII or above) at the Weld County Department of Social Services for twenty hours per week. This staff person would: A. accompany Intake Staff of the Child Welfare Division when they conduct follow- up visits for purposes of screening and identifying clients who would benefit from assessments for alcohol/drug intervention or mental health interventions. B. perform alcohol/drug intervention assessments as time permits, provide case consultation, case management, and in-house training on pertinent alcohol/drug issues as requested. 3. The Weld County Department of Social Services agrees to reimburse Island Grove at $55 per hour for on-site staff services at a total reimbursement level of$18,700.
